Description of Work: This position includes responsibilities for administering the
following programs: Adult Home Specialist, Representative Payee, At-Risk Case
Management, Adult Daycare Consultant (monitoring block grant funding), Housing and
Home Improvement, and Adult Protective Services. This position assures that various
types of adult care homes operate within the applicable rules, regulations and laws
through an ongoing planning and monitoring process. Activities are both consultative
and supervisory with the goal of assuring safe and quality care for the resident. The
social worker evaluates adult care homes (residential) and adult day care homes (day
care) to determine if the home or facility meets the needs of the individuals served. The
Social Worker also provides consultation and technical assistance to help maintain
standards, expand, and improve the quality of care provided. Position must engage the
adult and family in goals focused on improvement of quality of life for the elderly or
disabled person and enhanced safety. Position must deal with adult maltreatment,
extreme family issues (substance abuse, mental illness, domestic violence), potential
placement and care coordination, and participate in meetings through which very
intense temporary and/or permanent decisions are made for the best interest of the
adult. Excellent communication skills are required for interactions with staff attorney,
court representatives, court testimony, law enforcement, facilities, and other
professionals in the community. Employee must participate on a work team and have
strong, effective, efficient individual functionality, as well as support teamsmanship and
coordinated efforts. This position requires for workers to be on-call after hours every
other week for a full week (including nights and any holidays that fall within that time).
Additional after-hours responsibilities may be added when the unit is experiencing a
vacancy.
Education and Experience: Master's degree in social work from an accredited school
of social work; Bachelor's degree in social work from an accredited school of social work
and one year directly related experience; Master's degree in a human services field and
one year of directly related experience; Bachelor's degree in a human services field
from an accredited college or university and two years directly related experience;
Bachelor's degree from an accredited college or university and three years of directly
related experience
