QUALIFICATIONS 1. 2. 3. 4. High School Diploma or equivalent. Knowledge of medical office procedures, billing, and coding. Ability to communicate effectively with providers and clients. Typing and general office skills with advanced knowledge of computer and various office software including Microsoft Office. 5. Public speaking skills. 6. Personal vehicle and valid drivers license. 7. Ability to perform the functions of the position. DUTIES AND RESPONSIBILITIES 1. Develop provider referral network: a. Identify a minimum of 100 St. Lucie County health care providers/clinics that could refer clients to NDPP. b. Create spreadsheet with provider/clinic name, address, phone, email, website, and office manager or other point of contact. c. Prepare mailings to providers/clinics using materials developed by the Wellness Manager. d. Make follow-up phone calls to providers. e. Set appointments with providers/clinic staff to explain the program and referral system. 2. Educate providers: a. Meet with providers and clinic staff at their facilities to provide education. b. Make presentations about the program to individual providers, clinics, provider groups, hospital staff, and other community groups. c. Represent the program at community meetings as needed. 3. Manage referrals to NDPP: a. Collect and scan referrals from providers. b. Create client database to track program utilization. c. Call clients to confirm class times and dates. d. Mail reminder notices about class times and dates. e. Make follow-up calls to enrolled clients to promote attendance. f. Deliver materials to classes held in the community.
