The changes incorporated herein are an amendment to and supersede those conditions shown in the original solicitation. The Proposer shall acknowledge receipt of this addendum by signing this form and returning with their submittal. Failure to do so may subject Proposer to disqualification. Q1. I would like to know if this is the same bid as ITB-14-36-0-2014/KA that was awarded last July 3rd, 2014. Can you please provide me with more information on the process since we were ranked #3? A1. The contract for ITB 14-36 was terminated January 13, 2015, the District issued ITB 15-21 to solicit responses for security camera equipment. Public information requests may be sent in writing to the Purchasing Department to request any additional information. Q2. I called Human Resources this morning to clarify the requirements of the Jessica Lunsford Act, as stated in the bid. I talked with Liz Hensel there, and she checked and said that because I would not be going on school grounds to perform work, or be on school property alone with students, the Jessica Lunsford act would not apply to me. I told her I may, on occasion, go to the head school security office, but would be in the presence of a school security officer, and she said that would be all right. My question here is: do I note that on the bid? A2. Refer to GENERAL CONDITIONS, INSTRUCTIONS AND INFORMATION FOR BIDDERS, and page 26 for requirements for Jessica Lunsford. The information in the bid is to inform potential bidders of the requirements under Jessica Lunsford Act (Section 1012.465, Florida Statutes). Not all District vendors are required to hold the Jessica Lunsford badge. Q3. Since I will be making no deliveries, or working on school property, I was unclear why there is a worker's compensation requirement and an automobile insurance requirement. A3. Our standard insurance requirements are listed within the bid documents, any changes would have to be approved by Risk Management prior to contract award and Board approval. Q4. Do we have to include the below? There are so many different products here and some can be purchased without authorization from manufacturer. 1 School District of St. Lucie County ITB 15-21 “Must provide proof from the manufacturer that they are an authorized dealer and repair center for the equipment they are bidding.” A4. Yes, the District is requiring a manufacturer certification (for all items proposed). Q5. Is this solicitation strictly for equipment only, no installation? A5. Equipment Only. Q6. Near the end there is a checklist. One of the items on page 36 is "manufacturer's certification". I can find nowhere in the bid documents where this is explained. What would be the item(s) needed for this, and what does it mean? A6.
