Human Services Senior Practitioner– Adoption Social Worker Position Number – 2451BR This Human Services Senior Practitioner position is located within Child Welfare. Wake County Child Welfare has over 740 children in foster care. Currently, the Child Welfare division has over 315 children in foster care (in the legal custody or placement responsibility of Human Services) with a plan of adoption. This position is responsible for assessing and completing Pre-placement assessments regarding unlicensed relatives and families that are licensed by Wake County. This position will manage an adoption caseload and serve families within that case until the final adoption decree is issued by the clerk of court. This position is expected to have at least monthly face to face contact with each child and each pre-adoptive parent. This position is responsible for conducting post-placement visits with child (ren) and family; provide ongoing training and consultation; complete required adoption paperwork with the family and submit to clerk of court within required timeframes. This position is responsible for ensuring the adoption record is complete and entered into PRIME (WC electronic case management system) throughout the life of the case and into the appropriate database after the decree is issued. The position will access and monitor services to families ensuring the adoptive parents are able to effectively meet the needs of the youth placed in their home. This position will prepare reports to the court, attend court hearings when required and responsible for educating the adoptive family about post adoption supports. A valid driver's license and a "safe" driving record required. Preferred Requirements – Current NC Pre-Service Training for Child Welfare workers, public child welfare experience in a Dept. of Social Service in NC, and NC Child Welfare Collaboratives.
