April 18th, 2016 Dear Parent(s)/Guardian(s): My name is Tony Sexton and I am the boys' head tennis coach at Falcon Ridge Middle School. I have been a tennis coach here at Falcon Ridge for several years. I am very pleased your son has chosen tennis as a co-curricular activity. It is a life-long sport that teaches sportsmanship, integrity, and athleticism. Practices will be held every night! Please come up with a plan for your child to either get a safe ride/bike ride/ or walk home each night after practice. Practice ends at 4:45 pm and players are free to go home at this time. The following six things need to be done in order for your son to practice or play: First, your son needs a physical examination to participate in tennis. This form may be picked up in the school office. Physicals are now valid for three years and must be kept on file in the office. Second, please sign and have your son return the bottom section of this form indicating you have read this letter. Third, a $100 participation fee needs to be paid using FeePay (if appropriate- there are scholarships available). Fourth, You need to fill in and sign the "Fees, Permit, and Health Questionnaire" form from the main office at FRMS. ***This is a part of FeePay Fifth, your son must bring all these materials to the sports sign-up in Room 223. Your son needs to attend the mandatory meeting on Monday, April 18th after school (or see me individually to discuss these things) even if some of these things are missing, but he may not go on court until everything is turned in. Sixth, all players at the beginning of the season will tell their coach their own plan of how they will get home safely each night. All "Home" matches this year should start by 3:45 and SHOULD be completed by 5:30. We will return to FRMS from "Away" matches by 5:50. ****PLEASE On Time in picking up your son. Parents are allowed to give players other than their own child a safe ride home from FRMS only. Please feel free to come to any of our matches. I do ask that you let me know if your son will be riding home with you from any "AWAY" match. You will need to fill out a "liability release" form for all AWAY matches. Also, the only way you can get a ride home from a parent other than your own from an AWAY match is if you have a note that is signed by your parent and our athletic director, Mr. Roback. Mr. Roback is our Assistant Principal and he can be found in the main office. You must then give the note to your coach during the school day before the match. NO EXCEPTIONS.
