Dear Parent/Guardian,

Dear Parent/Guardian,
I am looking forward to the new opportunities the 2009-2010 school year offers. My wish is for
all students to reach their academic and personal goals this year. To help your student reach their
goals, I would like you to write a short biography about your child. Please include anything that you
believe would be useful for me to know in order to help them as individuals in my classroom. Their
experiences, successes, and struggles in school, especially math, will provide me with valuable
insight. This information can be as brief as two sentences or as long as you want.
Please return this assignment to me by Monday, August 31st. It is worth 10 points, but don’t worry
I won’t grade spelling or grammar. You may send this information via email, mail or your student.
If sending via email, please put your student’s name in the subject line of the email. Everything will
be kept entirely confidential.
I look forward to hearing from you and hopefully meeting you at Back to School Night August 24th.
Thank you!
