1. SCOPE OF ARRANGEMENT.

This Agreement for Services is for MOTEL ROOMS provided by the above named Vendor. This agreement becomes effective upon approval of Forest Service

Procurement Personnel and will remain in effect until cancelled by the Forest Service or the Vendor.

2. PERSONS AUTHORIZED TO PLACE ORDERS.

3. PROCEDURE FOR PLACING ORDERS.

Orders will be placed by telephone or personally as follows:

Crews : Reservations will be made by one of the above or their agent giving crew name, number in crew, number of rooms required, duration of occupancy, and estimated time of arrival.

Individual Reservations : Reservations will be made by one of the above or their agent giving individual's name, home unit, type of room required, duration of occupancy, and estimated time of arrival.

4. EXTENT OF OBLIGATION.

The Government is obligated only to the extent of authorized purchases made under this Agreement for Services.

5. PURCHASES UNDER THIS AGREEMENT.

Individuals . Forest Service will issue a lodging authorization form to each person authorized to occupy a room. This form will include the printed name and signature of the occupant, his/her home unit, and crew name. Occupant shall present such form as evidence of his/her authorization for occupancy. Vendor shall obtain a signature of each occupant on the room registration slip and shall attach the occupant's authorization form to such registration slip.

Crews . The Crew Boss will present a lodging authorization form and a list of crew members.

The vendor will obtain name of occupant on each registration slip. Registration slips and the lodging authorization form will be attached to the summary billing for the rooms.

Forest Service will only be responsible for lodging costs. Lodging costs DO NOT include phone calls, bar bills, restaurant bills, movie rentals, etc. Occupant will be responsible for all other charges to the room and Vendor will be responsible for collecting from occupant prior to or at check out. Any costs paid for by occupant will be shown on receipt.

6. BILLING AND PAYMENT PROCEDURES.

A summary invoice shall be submitted at least monthly or upon expiraton of this Agreement for Services, whichever occurs first, for all deliveries made during a billing period, identifying the registration slips covered therein and stating their total dollar value. The invoice shall be supported by signed registration slips and supporting lodging authorization form and crew manifest. The invoice will further include this Agreement for Services number. In times of heavy use under this agreement, summary billings may be submitted on a daily or weekly basis.

Payments may be made by any of the following methods at the discretion of the Government: a. Payment may be made by Credit Card (Government Visa Card) b. Payment may be made by Convience Check c. Payment may be made from a summary invoice that includes billing for all rooms furnished during the billing period. This summary billing shall include the above information and supporting documents. d. Under emergency these payments may be exempt from the Prompt Payment Act and may be paid upon receipt of a summary invoice and supporting documents by an Assistant Disbursing Officers

7. PRICING.

The government will not be obligated for motel rooms costing in excess of:

Single room - one bed - 1 person

Single room - one bed - 2 people

*If no single room is available, an individual may be housed in a double room at the above rate at government expense. If an occupant, however, demands a double room when a single is available, occupant will pay the difference between the single and double rates. Government will pay for only single room-single occupancy in this case.

Rates under this agreement may be negotiated if larger units are required at the time the order is placed.

Rates for all units will, however, not exceed those rates offered the Vendor's most favored customers.

Vendor's terms of payment are as follows:

8. GENERAL.

The attached Terms and Conditions are hereby made a part of this agreement. No conditions of sale other that as specified herein, or as may be mutually agreed to for a specific order, shall be applicable to purchases made under this agreement.
