Declaration of Commitment to Avoid Conflict of Interest
Individuals serving on the Alverno College Institutional Review Board are asked to affirm below
their commitment to (a) be sensitive to potential conflicts of interest and (b) to proactively take
appropriate action when a conflict of interest is identified.
I will not participate in an IRB's initial or continuing review of any project of any proposal for
which I (or any members of my immediate family) have a personal, financial, fiduciary, or
professional conflict of interest, except to provide information requested by IRB.
