Complete and submit this application to the Office of International Programs (Liberal Arts
North/Dugas, Room 402) by September 1 (type as much of your application material as possible
directly on this form).
*Secure the signatures of your Department Chair and Academic Dean before submitting to OIP.
OIP estimates the number and dollar amount of Study Abroad Scholarships based on information
from the planning proposals received by May 1. Scholarship funds may not be available for
programs whose Planning/Final Proposals are submitted after the due dates.

PROGRAM TIMELINE
Describe with as much detail as possible the city/country location as well as the academic, cocurricular, transportation, accommodation, meal arrangements, and major activities and visits,
including arrival and departure dates. A detailed itinerary (for each day of the program) should be
submitted no less than two weeks before the departure date. This timeline will assist
OIP/Providers with logistic arrangements.

SAFETY & EMERGENCY PLAN
Orientation/Student Preparation – In addition to the required OIP pre-departure orientation, you

must have a plan for providing students with information on safety, health, legal, environmental,
political, cultural, and religious conditions in the host countries, as well as expectations, planning
logistics and contingency plans.
Outline your plan for providing this information to students through pre-departure and onsite
orientations. To assist you with your orientation, review the CDC Traveler’s Health website
(www.cdc.gov/travel and country specific information provided by the U.S. Department of State
http://travel.state.gov/travel/).

DRAFT SYLLABUS (Attachment A)
Refer to the Forum of Education Abroad objectives outlined in the Planning Proposal form when
developing your syllabus. The syllabus must clearly demonstrate 15 contact hours for each
semester credit hour.
For each course offered during the program, attach a course syllabus that includes the following:
 Student learning objectives of the course;
 Assignments/activities that will be used to determine how well students attained the
learning objectives;
 Additional expectations for honors students and/or graduate students, if applicable.
IV.
BUDGET
a. Provide the program budget indicating the estimated final program cost per student and
the minimum number of students on which the budget is based. OIP will work with you on
the budget details using the OIP template spreadsheet. The current exchange rate should
be used when calculating expenses; however, an additional 15-20% should be included to
account for fluctuations in exchange rates and unexpected expenses. (Attachment B)

Innovation for Student Recruitment (please complete this section if you would like to be
considered for the ‘travel grant’ – this will pay for up to $3,000 of a faculty leader’s travel costs
associated with a faculty-led study abroad program, thus taking that amount off of the ‘per
student’ program cost)
How will you innovate to recruit students and design a program that will attract a wider range of
students? Special consideration may be given to new leaders and STEM courses. * Programs must
