This is the official record of the game used by the player agents and without it, they can't do their job of keeping up with wins/losses, pitcher
eligibility or other stats. Every component of this form is necessary for the proper administration of our league. The league, coaches AND
THE KIDS are counting on you to fill it out COMPLETELY and ACCURATELY. An example of a properly filled out form is attached to the
official book. If you have any questions, please refer to that or ask the field boss or your coach.
The game summary sheet is the responsibility of the home team. Please complete the game summary sheet and have the head coach from each
team sign it at the end of the game. Once the game summary is complete (and signed) please return it to the on-duty field boss.
