We believe
 In making healthcare available to the community we serve
by removing geographic, transportation, financial and
cultural barriers to service.
 Lack of insurance or inability to pay should not be a barrier
to obtaining screenings, immunizations, wellness checks or
general medical care.
 Chronic diseases can be prevented with available low or nocost screenings.
 Collaboration with Munson Healthcare will provide for
seamless patient care with technology, resources, referrals
and diagnostic services for our clients.
 All nursing staff shall practice according to standards of
practice using evidenced based care and while advocating
for patient health and wellness.

Objectives

To reach underserved people in the Grand
Traverse County area by delivering proven costeffective prevention activities helping to control
healthcare costs and reduce health disparities.
•
To develop a mobile health clinic to serve the
residents of Grand Traverse County by January
1, 2013.
•
Deliver healthcare services with a wellequipped mobile health clinic, decreasing
logistical and financial barriers to healthcare.
•
To foster community involvement in healthcare;
with appropriate referrals and follow-up
appointments for every patient.
•
Promote, advocate, and educate about health
and wellness in a confidential and consistent
setting using evidenced based nursing practice.
•
To deliver culturally respected and linguistically
sensitive health promotion, disease
management, and health education with
comprehensive care plans for every patient.
•
Nonbiased care provided by professionals
without discrimination in collaboration with
existing community services.
•
To provide healthcare services to every patient
regardless of insurance or ability to pay.
•
Providing committed and consistent service
with long-term solutions for a healthier
community.
•
To be fully operational interacting with people
at convenient locations with a full patient load
by March 1, 2013.

The candidate will provide nursing assessment, referrals for clients, work
effectively and efficiently with tight deadlines, high volumes and multiple
interruptions. Effective written and verbal communication skills required
with ability to make quality, independent decisions.
• Certifications required: ACLS, BLS
• BSN/Registered Nurse in the State of Michigan
• Unrestricted Drivers License
