INVITATION FOR THE PROVISION OF SERVICES
OF CERTIFICATION BODY THROUGH SMALL-VALUE PROCUREMENT
The Head Office Bids and Awards Committee for the Procurement of Goods (HO-BAC-PG) of the
Philippine Ports Authority, with office address at PPA Bldg., Bonifacio Drive, South Harbor, Port Area,
Manila, invites bidders for the Provision of Services of Certification Body for the Port of Cagayan de Oro.
The Certification body must be duly accredited by the Philippine Accreditation Office, Bureau of Product
Standards, Department of Trade and Industry, registered with SEC and have at least 5 years experience as certification body.
The scope of services of the Certification Body shall include the following:
(a) Certification Audit
(b) Issuance of ISO 9001:2008 Certificate Valid for three (3) years
(c) Surveillance Audit for the 2 nd year (Year 2012)
(d) Surveillance Audit for the 3 rd Year (Year 2013)
P 300,000.00
