National Chung Hsing University Regulations for Performance Appraisal and Reward of Service & Learning Craft Arts Education Grade Approved at the 36th University Affairs Meeting on May 15, 1999 Amended at the 4th Craft Arts Education Advisory Committee on March 21, 2001 Amended at the 8th Craft Arts Education Advisory Committee on June 16, 2005 Amended at the 15th Craft Arts Education Advisory Committee on February 22, 2012 Chapter I General Provisions Article 1 These Regulations are established in accordance with Article 13 of the "National Chung Hsing University Implementation Regulations on Students' Craft Arts Education". Article 2 These Regulations consist of such elements as application for leave, make-up and retake, performance appraisal and reward, et al. Chapter II Application for leave, make-up and retake Article 3. Application for leave and excused/unexcused absence: 1. Any student who fails to attend the Craft Arts Education class with cause shall apply for leave in accordance with the University's "Regulations for Students' Application for Leave", and shall submit the application to the Career Development Center for registration. 2. Students who fail to attend Craft Arts Education or to file an application for leave shall be deemed committing unexcused absence. 3. Personal leave and official leave shall be requested in advance. Sick leave or funeral leave shall be requested within one week upon expiration of the leave; otherwise, the student who takes the leave shall be deemed committing unexcused absence. 4. Students who attend Craft Arts Education shall not be late for school or leave early. Students who have been late for school or leave ten minutes or more early for three times shall be deemed committing unexcused absence for once. Article 4 Make-up course and exemption from make-up course: 1. The personal leave and sick leave requested by students during semester shall be no more than 10 hours in total. The application for leave shall be completed whenever the student takes leave, and the student shall make up the course. 2. Official leave: Make-up course may be exempted if the application for leave is completed in advance and the certificate of official leave is attached. 3. Funeral leave: Make-up course may be exempted if the application for leave is submitted attached with the certificate attached. 4. Students who shall make up the course but fail to do so without justified cause shall be deemed committing unexcused absence, and will not be granted a second chance for make-up course. 5. The make-up course shall be completed before the end of the semester. Students who fail to do so shall be deemed committing unexcused absence. 6. Article 5 Chapter III Upon completion of the make-up course after personal leave or sick leave, the record for excused/unexcused absence will be discharged, and the make-up course grade shall be no more than the minimum, 80 points. Retake and super senior 1. Students who have committed unexcused absence for three times or more in a semester shall retake the course. 2. Students who have committed excused/unexcused absence and taken personal leave/sick leave for more than 10 hours or more in total shall retake the course. 3. Students who fail Craft Arts Education in a semester (including those who still fail the course after making up it) shall retake the Craft Arts Education course in whole in that semester. 4. Students who need long-term recuperation for hospitalization or injury/disability (consecutive leave) shall apply for an extended study. 5. Students who apply for retaking the course or extending study shall take the full semester as the basis. The hours of Craft Arts Education in the previous semester will not be combined. Performance appraisal Article 6 Academic performance valuation: 1. The personal fundamental grade per student of the Craft Arts Education shall be 80 points. 2. The academic performance evaluation list for Craft Arts Education consists of three elements, including Punctuality (0~minus 10), Cooperation (10~minus 15), and Performance (10~minus 15). The scores of each element might increase or reduce per the performance, which will be the scores for the given week plus the fundamental scores. The average academic semester scores minus the excused/unexcused absence scores will be the preliminary grade. The tutor of Craft Arts Education may add or deduct the student's grades per his/her performance, and then give the final course grade. Article 7 Assessment on scores deducted for being late for school, leaving early and excused/unexcused absence: 1. Deduct the total final course grades by 8 points per excused/unexcused absence from the Craft Arts Education course. In the case of excused/unexcused absence for three times or more, no final course grade will be given. 2. The "Punctuality" section of the student who is late for school or leaves early shall be identified as 0~minus 10. Article 8 Grade entry: 1. The craft arts section chief shall enter the preliminary evaluation scores in the evaluation form subject to each team member's status of the crafts art education. The re-assessment shall be conducted by the tutor of Craft Arts Education by adding or deducting scores of the students subject to the students' performance. 2. The final course grade for Craft Arts Education shall be evaluated by the tutor of Craft Arts Education once within one week prior to the mid-term exam and 5 final exam in each semester. Once the academic performance evaluation form for Craft Arts Education is signed by the tutor, it shall be submitted to the Career Development Center of the Office of Student Affairs prior to the final exam for compilation and registration. Chapter IV Reward Article 9 In order to praise the excellent students who participate in the Craft Arts Education, the tutor will recommend 3 excellent students from each group in each semester, who will be praised in public and awarded the certificate of merit for Craft Arts Education. Article 10 Students with full-attendance in the whole academic year (two semesters) may be awarded the certificate of merit for full-attendance to Craft Arts Education. Article 11 Students who have applied for leave (exclusive of official leave), committed excused/unexcused absence, been late for school, left early, or made up courses shall not be awarded the full-attendance reward. Article 12 The Craft Arts Education grade may serve as the reference for the application for scholarship, selection of excellent students and other rewards. Chapter V Bylaws Article 13 These Regulations shall be enforced upon approval of the Craft Arts Education Advisory Committee meeting. The same shall apply where these Regulations are amended. 6
