Primary Care Physician
Triage Nurse – Fifth Street Site
Esperanza Health Center seeks a Primary Care Physician
(Primary Care Physician, board-certified family
physician, med-peds or internist) to provide care for
adults only, or adults and children. Work in PatientCentered Medical Home holistic team ministry model
with support staff, behavioral health consultant,
nutritionist, social worker, and chaplain. Ability to speak
conversational Spanish is required, or willingness to
learn in a short-term intensive immersion setting.
Evaluate patients four days per week and one Saturday
per month, plus occasional night call coverage from
home. Providers have received loan repayment via
National Health Service Corps, the Pennsylvania Loan
Repayment Program, or MedSend.
Esperanza Health Center seeks a full-time Registered
Nurse to provide triage nursing care to patients from
the Latino and underserved communities of North
Philadelphia. The position is part of the Primary Care
Team that provides holistic Christ-centered care to our
patients. General duties include: assess needs of walk-in
and call-in patients, participate in patient health
education, coordinate multidisciplinary referrals and
follow-up of more complex patients, provide spiritual
care, and perform basic Medical Assistant duties as
needed. Provide direct supervision to medical assistants
at Esperanza’s 5th Street location.

Education, Skills, and Experience:
 RN with a minimum of one year post-graduate
experience, preferably in a hospital setting

HIV Nurse Care Coordinator
Esperanza Health Center seeks a full-time Registered
Nurse for the HIV Care Coordinator position. This
position is responsible for coordinating and providing
primary care services to patients with HIV disease, in
collaboration with the center’s medical staff. Maintain
individualized documentation/care plan for HIV patients
based on on-going assessment and flags services
needed. Provide on-going patient education to
empower patients to take an active role in managing
their health. Work closely with the broader HIV team to
assure that patients receive needed services and
referrals. Assist with HIV testing activities in the clinic.
Education, Skills and Experience:
 Licensed Registered Nurse
 Professional nursing experience in a hospital or
community setting
 Experience working with patients with HIV/AIDS;
 Bilingual in Spanish and English; ability to
communicate professionally in written and verbal
form

Billing Manager
Lead Nurse Care Manager
Esperanza Health Center seeks a full-time Billing
Manager. The Billing Manager is responsible for the
overall billing operations including medical and
behavioral health billing. The Billing Manager will
supervise and train staff members, as well as work
cooperatively with the Assistance Finance Director and
other management staff. General duties will include
leading and directing the delivery of revenue cycle
management for the organization through management
of direct reports in various functional areas including
coding, preparing and submitting claims, payment
posting, denial management, and AR follow up, as well
as performing daily billing activities.
Esperanza Health Center seeks a full-time Lead Nurse
Care Manager. The Lead Nurse Care Manager is
responsible for managing a program to decrease
emergency room utilization and hospitalizations of
identified patient populations. The Lead Nurse Care
Manager will lead the disease management outreach
team, focusing on high-risk and/or high-utilizer EHC
patients who require multidisciplinary case
management. The Lead Nurse Care Manager will
develop and implement care plan strategies and patient
education during in-home visits, collaborating with EHC
team members to provide linkages for the various
health and social needs of patients. The Lead Nurse
Care Manager coordinates the care management
programs at Esperanza Health Center.
Education, Skills, and Experience:
 Three to five years of medical billing expertise in a
primary care environment are desired
 College degree or relevant experience preferred.
Advanced working knowledge of CPT, ICD-9, HCPCS
is required. FQHC billing knowledge is a plus
 Strong organizational, interpersonal, written
communication and computer skills are required
Nurse Care Manager
Esperanza Health Center seeks a full- time Nurse Care
Manager to provide comprehensive wellness visits and
improve data sharing with insurance companies. The
RN Care Manager provides holistic care management to
improve health outcomes and decrease emergency
room utilization and hospitalizations of identified
patient populations. The RN Care Manager will develop
and implement care plans and patient education,
working with team members to provide linkages for the
various health and social needs of patients. This
position is a temporary 12-month position, with the
possibility of continuance beyond that period.
Education, Skills and Experience
 Registered Nurse license required; college
graduate (BS RN) preferred
 Bilingual in English/Spanish, or willingness to

Integrated Health Services Coordinator
Food Service Worker/Driver
Esperanza Health Center seeks a full-time Integrated
Health Services Coordinator to provide support to
behavioral health and other patient care departments in
program development, reporting quality assurance,
enhancing patient care through data collection and
evaluation, and research. The Integrated Health
Services Coordinator will help to facilitate and
coordinate centering groups such as Pregnancy and
Parent Centering groups for Esperanza’s patients
including patient recruitment and follow-up. Other
duties include organizing departmental meetings as
directed by the Behavioral Health Director;
collaborating with outside community mental health
agencies; and occasionally representing Esperanza at
off-site meetings which may include evening hours.
Esperanza Health Center seeks a part-time Food Service
Worker/Driver position. The Food Service
Worker/Driver performs the food preparation,
customer service and kitchen sanitation tasks.
Additional responsibilities include driving and
purchasing goods and supplies; stocking café supplies;
operating the cash register; monitoring the functioning
of equipment; and opening and closing the café for
operation depending on the shift. Must use own insured
car; and will be reimbursed for mileage. This position is
23 hrs/week.
Education, Skills and Experience:
 Bachelor’s degree preferred with 1-3 years of
public health, program management,
counseling or related experiences.
 Proficient in Microsoft Office and Data
Analysis; prior experience with Access or other
database system preferred
 Experience developing programs and
conducting trainings and or facilitating group
sessions
 Proficiency in Spanish required
Social Services Case Manager
Esperanza Health Center seeks a full-time Social
Services Case Manager. This position is designed to
address the bio-psycho-social-spiritual needs of EHC
patients and the community which predispose illness or
interfere with obtaining maximum benefit from medical
care. The Social Services Manager’s responsibilities
include social work, case management and HIV testing
& counseling in a comprehensive, interdisciplinary
approach to client-centered care.
Education, Skills and Experience:
 High School Diploma/GED and one year
professional experience in commercial food
preparation, or equivalent combination of
education and experience
 Must be ServSafe certified or be willing to
obtain at own expense within 90 days of
employment
 Bilingual English and Spanish, preferred
 Must have a valid PA driver’s license, personal
vehicle and vehicle insurance coverage
Physical Demands:
 Must be able to carry or move up to 40 pounds
unassisted
 Must be able to stand for 6 hours at a time
working in a fast paced environment with
occasional stooping, bending, and kneeling
 Requires the ability to work in warm and humid
conditions
Education, Skills and Experience:
• Master’s or Bachelor’s Degree of Social Work
preferred. An equivalent combination of education and
a professional experience will be considered.
