● PATH = Providing Access to Healing
● Provide a master’s level therapists at
each school throughout the school
year in order to provide consistency
in therapy and build relationships
with students, parents, teachers, and
other school personnel.
Fox City United Way Path Program
Offer mental health therapy to students in the target
population as referred by participating school districts and
provide the following core services:
● Assessment
● Treatment
● Communication - with school personnel being
served by the program
● Staff Development
● Necessary referrals to psychiatrists, primary care
physicians, and other provider organizations as
needed.

Program
Day Treatment Program:
Level II Day Treatment - as defined by State
•
Full Day Program
• Two Age Tracks 7th - 9th, 10th - 12th
• Multidisciplinary, Holistic Approach - focus on
mental health and physical wellness
• Strong Family Component
• Case Management and Care Coordination
The day treatment curriculum uses socialization and recreation
therapy in addition to individual, family, and group
psychotherapy.

The purpose of the day school is to
provide an educational opportunity for
students coming from intensive in and
outpatient services in conjunction with
Catalpa Health day treatment.
