Step-by-Step Instructions for the Creation, Revision,
or Deletion of a Course
To Propose a Newly Created Course
Step 1
Use the Course Outline Template to prepare a course outline for the proposed course.
Step 2
Prepare a Course Change & Addition Form.
 Check the box for New Course at the beginning of Part I.
 Fill out the form completely; all parts are required
Step 3
Have the Registrar review the form. The Registrar’s signature is required at the
bottom of the form.
Step 4
Have the ADAA review the form. The ADAA’s signature is required at the bottom of
the form.
Step 5
Prepare a Course Approval Form. Just fill out the information required in the text box
at the top of the form:
 Course Number and Title
 Date Submitted
 New, Revision, or Deletion (check one)
 Effective Date
 Submitted by
 Division or Area
Signatures for the Course Approval Form will be obtained through the remainder of
the approval process.
Step 6
Meet with your division in order to receive divisional approval for the course
proposal documents. The Approval of Division on the Course Approval Form must
be signed by a member of the faculty within the division of the proposed course, but
of a different discipline. (In other words, an anthropology instructor may sign for a
PECO or a PSYC course, but not an ANTH course. A political economy or a
psychology instructor must sign for an ANTH course.)
Step 7
Prepare a Course Creation Form for CAMS. Fill out the form completely; all parts are
required.

Step 8
Submit the course outline along with the Course Change & Addition Form, Course
Approval Form, and Course Creation Form for CAMS to the Registrar. The Registrar
will bring the course proposal documents to the Curriculum Committee for
consideration for approval.
When the course proposal is placed on a Curriculum Committee agenda, the
proposer will be notified of the date and time of the meeting. The proposer, or an
appointed substitute for the proposer, must be available for this meeting.
If the Curriculum Committee approves the creation of the new course, the
Curriculum Committee will obtain the remaining signatures for the Course Approval
Form. The documents will then be forwarded to the registrar for processing.
To Revise an Already Existing Course

Use strikethroughs for deletions and underlines for additions if you’re changing text.

Registrar will bring the course revision proposal documents to the Curriculum
Committee for consideration for approval.

To Delete an Existing Course

 Check the box for Course Deletion at the beginning of Part I.
 Completion of the A and B sections is required. C is only for newly created or
revised courses.
 Complete Part II, no. 3
 The rest of the form (except for the required signatures) does not need to be
completed. Those parts are only for newly created or revised courses.
