DRESS CODE: The uniform is to be clean, professional, washable, hemmed, includes the name tag, and is to be worn at all
times or the student will be dismissed from class. Uniform fabric is smooth, hard surface, and excludes jeans, sweats, corduroy,
denim and pile.
Smock Top: Black apron style (Style #975) or Black zip –up, button, or snap -up jacket that can be purchased at Sally’s Beauty
Supply in Mt.Pleasant.
Skirt or Slack: Solid black dress style. Skirt length mid knee to mid-calf.
Shoes: Black professional closed in oxford with low heel and crepe sole. No cotton or canvas shoes, or no boots.
Shirt: One of the following is to be worn under the smock. Black or White Tee Shirt with sleeves or, Blouse with collar and
sleeves, or Black or White Turtleneck. No tank tops and no writing on the shirts.
Hose: Natural with skirt style. Solid black or white socks may be worn with slack style.

MISSION STATEMENT:
The mission of M.J.Murphy Beauty College is to prepare cosmetology professionals for state license and a
rewarding career in the field of Cosmetology, Manicuring or Instructing.
ADMISSION REQUIREMENTS:
COSMETOLOGY and MANICURING – High school graduation diploma, or General Education
Development (GED) certificate. We do not admit Ability to Benefit Students. For Home Schooled Enrollee
a State Issued Credential must be provided.
RE-ENTRY POLICY
RE-ENTRY POLICY FOR A STUDENT WHO WITHDREW IN GOOD STANDINGS (OUT LESS
THAN FIVE MONTHS).
The student who was meeting the Satisfactory Progress standard at the time s/he dropped and applies to reenter within five months of the drop date will be given credit of all MPA’s theory hours and test accrued at
the time of the drop. A new contract will be written.
RE-ENTRY OF A STUDENT THAT DROPPED OR WAS TERMINATED WHILE NOT MEETING
THE STANDARDS OF SATISFACTORY PROGRESS:
A student that dropped, or was terminated because of not meeting satisfactory progress are not to be reenrolled unless they file an appeal and the appeal is granted. An appeal would only be granted if
circumstances that caused the student to not meet the standard have changed to the extent that the appeals
committee is convinced the student can now meet the standard.
ADDITIONALLY:
1) A student who was terminated because of discipline problems will not be re-entered.
2) The student who dropped or was terminated because of/ or while not meeting the MPA.
3) A student who was dropped or terminated because of/or while not meeting the theory grade
requirements would be required to take the entrance exam and pass with a 85% all of the theory test
needed to bring the student into compliance with the theory requirements of the standard of satisfactory
progress.
4) A student who was dropped or was terminated because of/or while not meeting the attendance
requirements of the standard of satisfactory progress, will be re-enrolled with the percent of attendance
on record from the previous enrollment. The student will be placed on probation for 3 months and will
be required to attend according to the contract rate of attendance ( approximately 80%). A chart will be
issued to the student so s/he can monitor the rate of attendance from the time of re-entry. The student
will not be issued any financial aid unless they are meeting the contract rate of attendance at the of 3
months.
INSTRUCTOR - Must hold current Michigan cosmetology license and have high school
Diploma or GED.
ALL COURSES - Students must present proof of required age and education; and must submit a fee of
$15.00 for State Registration. The school will register the student with the State Board of Cosmetology.
All courses will be taught in English.
GRADUATION FROM ALL COURSES:
Upon completion of the course hours, MPA’S (Minimum Practical Applications) & Practical Pre- State
Finals with a required passing grade of 75%. , Unit tests & Pre – State Written Finals with a required
passing grade of 85%. Written Assignments & payment of tuition and fees in full, the student will receive
a diploma attesting to graduation.

ORGANIZATIONS:
American Association of Cosmetology Schools, Michigan Cosmetology Schools Association, Michigan,
National and Midwest Student Financial Aid Associations. Administration and or staff members participate
in the National Cosmetology Association, Michigan Cosmetology Schools Association Instructor
membership.
HOLIDAYS OBSERVED
New Years, Memorial Day, Independence Day, Labor Day, Thanksgiving, And Christmas, contingent upon
what day of the week a holiday falls, the school may be closed for that holiday and the day following.
NON-DISCRIMINATION
This institution and its admission, instruction, and graduation policies, practice no discrimination on the
basis of race, color, religion, sex, age, or ethnic origin.
TRANSFER
The school only accepts transfers for geographic reasons. Students required to take the written and
practical entrance exam because of re-enrollment or transfer will be charged a $75.00 exam fee.
The school does not recruit students who are enrolled or attending another school of Cosmetology.
EMPLOYMENT ASSISTANCE
The school will assist graduates efforts to secure employment in the program completed or related field that
includes personal appearance guidelines job referrals and follow up.
We will assist all graduates who request placement assistance, without cost to the graduate or employer, by
posting job openings on the student bulletin board. The school, however, does not guarantee employment.
Latest available information on student retention and placement is available upon request from the school
manager.
FINANCIAL AID
Financial Aid is available to qualified applicants. TITLE IV Funds (outright gifts of money that do not
need to be repaid. A separate pamphlet entitled CONSUMER BOOKLET AND FINANCIAL AID
INFORMATION accompanies this catalogue or is available from the school manager and explains the
available program. A prospective student may log in to www.fafsa.ed.gov and apply for Title IV.
SCHOOL CLOSINGS
If the school is closed for unexpected reasons such as inclement weather that information shall be
announced on the radio station whose call letters are posted on the student bulletin board.
ADVISING
Academic, career and individual referral advising are provided to attending students. A student is referred
to a local counseling center (Listening Ear Crisis Center @ 989-772-2918). A student who is referred, a
written documentation is kept in the students file.

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT (FERPA)
An eligible student and parents/guardian of a dependent minor has a right to: Inspect and review* the
student's educational record, request an amendment to ensure the record is not inaccurate, misleading, or
otherwise in violation of the students privacy or other rights. Consent to disclosures of personally
identifiable information contained in the student's educational records, information will only be
released to a third party upon the written consent of the eligible student or parent/guardian, except to the
extent that FERPA and the regulations authorize disclosure without consent.
The school provides access to student and other school records as required for any accreditation process
initiated by the school or by NACCAS. File a complaint with the U.S. Dept of ED concerning alleged
failure by the school to comply with FERPA requirements.
A copy of the school FERPA policy is on file in each school manager's office, where it can be reviewed
and inspected, or a copy can be obtained by requesting in writing to the school's administrative office
(address listed in this catalogue). * By appointment at a mutually agreed upon time.
ABSENCES/ MAKE UP
A full time student can attend an average of 30 hours a week and still graduate by the ending date of the
contract. Excused absences will be given for illness or pre-arranged vacations. Students are required to
call in if they are going to be tardy or absent. Following ill absences a physician's verification is required.
When the student's absence exceeds that average for two successive two-week periods the student will be
warned in writing. If within the next two-week period the absences exceed the average the student may be
suspended for one week subject to confirmation by state board. Saturday attendance is required except for
pre-arranged vacation, or illness verified by a physician. Anytime the student accrues more than four
unexcused Saturday absences s/he will be suspended for one week. Suspensions will not extend the length
of the contract. A student returning from a leave of absence is responsible for making up any work
according to the MPA schedule. Management has the right to make exceptions to this policy.
ATTENDANCE & TARDINESS
Cosmetology is a clock hour course, and each hour must be used for meaningful learning activities.
Students must attend all scheduled theory and demonstration classes, and bring pencil, paper, and text to
class. Students must sign and clock in each morning; for lunch and breaks; and when leaving for the day.
Students are not allowed to leave the classroom without permission. Free time between patrons will be
used for studying only materials related to cosmetology or mannequin practice. Students not using optional
time productively will be required to sign and wand out. The student lounge is to be used only by students
signed out on break or lunch. Students signing and clocking in tardy after role call must sign in at the next
quarter hour and will not enter the clinic or classroom until that sign in time. When the student has been
tardy 3 times a written warning will be issued. The student will be suspended if the tardiness continues.
The student and/or her (his) parent or guardian hereby give the school permission to call upon a physician
or dentist and/or to refer the student to same for medical/dental assistance in the event of sudden illness or
accident. It is understood and agreed that any expense incurred will be the sole responsibility of the student
and/or her (his) parent or guardian
CONDUCT / TERMINATE ENROLLMENT
The student agrees to abide by the school rules, regulation, and dress code provisions in the pub-lished catalogue; regulations of the State Board of Cosmetology; to attend all classes regularly and
promptly except for sufficient reasons. The student agrees to park in the designated parking area. The
student shall not remove school property from the premises without written permission. A student shall not
use profanity, nor be insubordinate or disrespectful of staff members, other students, or clientele. The
school reserves the right to terminate any student whose conduct reflects discredit on the professional and
ethical standards of the school and/or the State Board.

The student and/or his/her parent or guardian hereby gives the school permission to call upon a physician
or dentist and/or refer the student to same for medical/dental assistance in the event of sudden illness or
accident. It is understood and agreed that any expense incurred will be the sole responsibility of the student
and/or his/her parent or guardian.
We now offer Part – Time Classes along with Full- Time Classes. Part Time Classes are 20 hours a
week . You pick your schedule of hours & days that you will be here 20 hours a Week. Full Time
hours stay the same at 35 hours per week.

If the student has not completed attendance requirements by the ending date as described above, an
additional charge per hour will be be payable until the hours are completed. This charge will be at
$7.00 per hour, billed upon reaching the date the student was to complete the hours.

CANCELLATION AND SETTLEMENT
The enrollment agreement, which there is no application fee, if accepted by the school, becomes an agreement which states all
the conditions of enrollment and is not subject to alteration or cancellation except as follows: (A) An applicant not accepted for
training by the school shall be entitled to a refund of all monies paid. (B) If a student (or in the case of student under legal age,
his/her parent or guardian) cancels his/her enrollment and requests his/her money back in writing, within three business days of
the signing of an enrollment agreement or contract, all monies collected by the school shall be refunded. The postmark on
written notification will determine the cancellation date, or the date said information is delivered to the school administrator in
person. This policy applies regardless of whether or not the student has actually started training. (C) If a student cancels his/her
enrollment after the three business days after signing but prior to entering classes, he/she shall be entitled to a refund of all
monies paid to the school less a registration fee of $200.00 for the cosmetology, manicuring, and instruction course., (D) for
students who enroll in and begin classes, the following schedule of tuition adjustment will be considered to meet minimum
standards. (E) All refunds are calculated on actual hours.

The school reserves the right to suspend any student whose tuition payments are in arrears until the payments are current
according to this contract. All tuition must be paid in full by time the student accrues these clock hours.
Cosmetology Course -1500 hrs Cosmetology Instructor Course – 500 hrs Manicure Course – 400hrs
Pertaining to Rule 39 of the State Board of Cosmetology it is reasonable for a school to withhold the release of a transcript or
state board examination of a student when unpaid fees of tuition to the school is due. Unless under justifiable circumstances the
Department will no longer process a certification of hours for a student who cannot get their hours from an existing school if
they are delinquent in their payments.
E) Enrollment time is defined as the time elapsed between the actual starting date and the date of the students last day of physical
attendance in the school. Any monies due the applicant or student shall be refunded within 45 days of formal cancellation by the
student as defined in item (B), or formal termination by the school, which shall occur no more than 45 days from the last day of
physical attendance, or in the case of a leave of absence . Students who do not return from a leave of absence, last date of leave
is used as the termination date. Withdrawal of a student on a leave of absence shall be the earlier of the date of expiration of the
leave of absence or the date the student notifies the institution that the student will not be returning.
(F), A list of, and cost to the student, of required kit, equipment, books and supplies is listed in the published catalogue. The kit
may be purchased either from the school or from a retail beauty supply house. The cost of the kit and supplies (except as stated
in A, B and C above) is non-refundable, and is not included in the tuition adjustment computations. These items become the
property of the student when issued; but shall remain in the school until the students completes the course or terminates, and
until paid. Any lost equipment must be replaced at the student’s own expense. Students who terminate prior to course
completion will lose the non-refundable $200.00 registration fee and will be charged a $100.00 termination fee. (G) If a school is
permanently closed and no longer offering instruction after a student has enrolled, the school will provide a pro rata refund of the
tuition to the student. (H) If a course is canceled subsequent to a student's enrollment, and before instruction in the course has
begun the school shall at its option: provide a full refund of all monies paid; or provide completion of the course. (I) If the
student is receiving TITLE lV Funds and training has begun when the course is canceled the school shall calculate a Return of
Title IV using R2T4 Refund Policy. All other refunds will be calculated using the Institution Schedule.

OBJECTIVE
The objective of the Instructor Course is to prepare the cosmetologist for a successful and rewarding career
as a cosmetology instructor. As a result of the 500-hour course of theory and practical training the student
instructor should gain the knowledge, skills, and ability to competently:
