Final Report Guidelines and Form
As indicated in your grant award letter, The Nuestro Futuro initiative of The Chicago
Community Trust requires your organization to submit a Final Report including
narrative and financial information on activities related to the grant. The Trust
emphasizes the importance of Final Reports, as they are an integral part of our
monitoring and evaluation process. They are not only necessary for proper oversight
to ensure accountability, but also serve as a valuable learning tool for the Nuestro
Futuro Board, Trust staff and your organization. We greatly appreciate your candor
and thoughtful review of your project experience. Be assured that we do not share
these documents with any organization or funders.
Your completed report should be emailed to grants@cct.org and must be received
by <due date>. Please attach this report to your e-mail as a Word document only.
Any supplemental materials may be emailed to grants@cct.org or mailed to Sandy
Phelps, Grants Manager, The Chicago Community Trust, 111 East Wacker Drive,
Suite 1400, Chicago, IL 60601.

Final Reports include a Narrative and a Financial Report, and are due on or
before the due date indicated in your grant record form. Once reports become
past due, applications currently submitted and/or future funding considerations
will be affected.
Final Reports are an indication that your project is complete and that all grant
funds have been spent. However, if your project is not complete or any grant
funds remain unspent at the time Final Report is due, please do not submit any
reports, but rather, contact Grants Management at grants@cct.org or 312/5652311 prior to the reporting due date.
Always download the form from our website www.cct.org to ensure you are
submitting the most current version.
o Narrative Report:
 Answer all questions directly on the report.
 The final narrative report should present a review covering the
entire grant period. If your project was not completed at the time the
Grant Report is due, do not submit any reports: please contact

pertaining to line items not shown on the form should be listed under “Other”
and noted in the narrative report.
