SECURITY Once you have entered MyUH, your personal session and academic records are available to anyone using this computer. For your
security, please close (or ‘quit’) your web browser before leaving, especially if you are using a public computer.
REGISTRATION, PAYMENT, AND POLICIES
You bear full personal responsibility to register for the correct courses and for meeting all course prerequisites and requirements. You must
process any course changes prior to the published drop/add deadlines and adhere to all University policies and procedures. Tuition and fees for
courses not dropped during the 100% refund period will be assessed according to the refund schedule of the campus that offers the course.
You are responsible for payment of tuition and fees incurred because of registration. By registering for classes, you agree that you are accepting
financial responsibility for payment of charges for educational services. These charges may include but are not limited to tuition, student activity
fees, course and other registration fees, room, meal plans, and collection costs that the University of Hawai’i incurs in the collection of unpaid
balances.
Full payment must be received on-line or at any campus Cashier’s Office by your payment deadline. Please note that while the University
reserves the right to cancel the registration of financially delinquent students, your registration may not be automatically cancelled for non-payment
of tuition and fees. It is your responsibility to avoid charges by canceling your registration prior to the first day of classes each semester. By
canceling your registration before the start of classes, you will avoid any charges and your class spaces will be released in time to accommodate
other students.
If you provide a cell phone number, you authorize the University of Hawai’i and its agents to contact you at that number and/or at any other
cellular telephone number or wireless communication service you provide in the future, using automated telephone dialing systems, artificial or prerecorded voice or text messages, or personal calls, regarding your obligation to repay any debts owed to the University resulting from registration.
You are responsible to review your payment profile in the Student Account Homepage at least once per term to ensure information is current.
RECORDS OF ON-LINE TRANSACTIONS Your use of your UH username and password to conduct transactions with the University will result in
official University records of your academic and financial activities and responsibilities.
ELECTRONIC DELIVERY OF IRS FORM 1098-T TUITION STATEMENT FOR EDUCATION TAX CREDIT You consent to receive your Form
1098-T Tuition Statement tax form electronically. For more information on Form 1098-T visit http://www.fmo.hawaii.edu/bursar/1098tconsent.html
USE OF EMAIL You understand email is an official means of communication within the University and your hawaii.edu email address may be used
for notification of legal action, financial notice, and academic or disciplinary action such as academic warning, probation, suspension, provided
appropriate confidentiality and privacy of records are strictly maintained.

INSTRUCTIONS for Outreach College Credit Course REGISTRATION AND PAYMENT FORM
Students are usually required to register for their course online through MyUH (http://myuh.hawaii.edu).
GENERAL INSTRUCTIONS
The purpose of this form is to allow paper registration.
All registration forms will be processed as time permits. In cases when registration errors and/or holds prevent the processing of your registration,
we will send an email to your UH Email account notifying you of the problem. You are responsible for obtaining the appropriate approvals and/or
removal of holds and for notifying us when to continue processing your registration.
You must satisfy all course prerequisites, including placement tests, as listed in the UH Manoa Catalog (www.catalog.hawaii.edu) to enroll in the
specified courses. For courses numbered 500–800, you must have a U.S. bachelor’s degree from a regionally accredited U.S. college or university
or equivalent. 500–level courses are considered post-baccalaureate but are not applicable towards a degree.
ACADEMIC CALENDAR: Refer to the course academic calendar for the last day to Add, Drop, Withdraw, and the 100% and 50% refund dates.
Please be sure to check the Outreach College website (www.outreach.hawaii.edu) for the academic calendar of the course you wish to register for.
You are responsible for knowing the deadline dates for refunds and withdrawals.
SUBMIT: Turn in this registration form with payment to Outreach College Student Services by postal mail to 2440 Campus Road, Box 447,
Honolulu, HI 96822 or in-person at Sinclair Library Room 301, by fax to (808) 956-3752, or by email as an attachment to ochelp@hawaii.edu.
Registration forms must be post-marked and/or received by our office no later than the dates listed on the course academic calendar, unless
otherwise specified.
CONFIRM REGISTRATION: Please check your MyUH account (http://myuh.hawaii.edu) to confirm your registration. NOTE: The Cashier’s Office no
longer prints receipts. Students can print their own receipts by logging onto the MyUH payment option.
CHANGES OF REGISTRATION AND WITHDRAWALS: You are to make changes to your registration on MyUH (i.e. drops, adds, and/or changes
in grade options) by the appropriate deadlines as noted on the course academic calendar.
Fall/Spring Drops: After the term begins, MyUH will not allow you to drop your last or only Extension term course. Email a request to drop your
last class to ochelp@hawaii.edu from your hawaii.edu email by the “last day to drop”.
Fall/Spring Withdrawal, “W” on record: You cannot withdraw from a Fall or Spring Extension term course on MyUH. To withdraw between the
"last day to drop" and the "last day to withdraw", contact Outreach College Student Services at (808) 956-7221 or send an email to
ochelp@hawaii.edu from your hawaii.edu email. Instructions will be given to you. You may be required to obtain the Instructor's written approval.
Some colleges also require the Academic Services Dean's approval.
Summer: You may drop and/or withdraw from summer classes using MyUH until the last day to drop and/or withdraw.
GRADE OPTIONS: Deadline to change grade options (letter grade to CR/NC or CR/NC to letter grade) is the same as the last day to add the
course.
A – F (Letter Grade): This means that you will be assigned a (plus/minus) A, B, C, or F grade and will have the grade computed in your grade
point ratio. If you plan to earn a degree and want this course to be applied to your degree, you should take this option.
CR / NC (Credit / No Credit): This means you will be assigned a CR grade if you earn a grade of A, B, or C and a NC if you earn a grade of C-,
D, or F. This grade will not be computed in your grade point ratio. Education courses numbered 500–599 are offered for the CR/NC and
AUDIT options only.
Audit (“L” Grade): This means you will not be assigned a grade. You must obtain an Audit Approval Form with the written approval of the
instructor and submit the signed form by the last day to add the course. NOTE: Tuition is the same regardless of whether courses are taken
for letter grade, CR/NC, or Audit.
HOLDS: If you have a HOLD on your account, you will not be able to register or obtain a transcript or diploma. To find out if you have a HOLD, log
onto MyUH and click on “Check Registration Status”. If you have a HOLD, it will be listed as well as the department that placed the hold. For an
academic HOLD, you must go to the respective office to clear the HOLD. For a financial HOLD (i.e. bounced check, library, or parking fines), you
must go the respective office to settle your account. The financial obligation must be paid, then bring your receipt of payment to the NonRegistration window (#2) at the Cashier’s Office, Queen Liliuokalani Center for Student Services, room 105. The HOLD will then be removed from
your record and your desired transaction will be processed.
REFUNDS: For check and cash payments, your refund will be mailed to you, or sign-up online to deposit your refund directly into your bank
account. View eRefund information at http://hawaii.edu/news/docs/eRefunds.pdf. For charge payments, your refund will be credited to your charge
account and reflected on your monthly statement. Course Administrative Fee, lab and late fees are non-refundable and non-transferable.
Please allow approximately four to six weeks for refund processing.
RETURNED CHECKS: If your check is returned for ANY reason, you are assessed a $25.00 returned check fee and a HOLD is placed on your
account. Interest on the $25.00 returned check charge is assessed at the rate of $0.10 per month or a fraction thereof, for the period beginning the
first calendar day after the date of notification from the bank at which the check has been dishonored, to the date paid. If payment has not been
made within 10 days, your registration may be cancelled.
Do NOT STOP PAYMENT on checks. A stop payment on a check is regarded as a returned check and is not acknowledged as an official drop
from the class or withdrawal from the course.
The University of Hawai’I and Outreach College are not responsible for any cancellation/change charges assessed by airlines or travel
agencies. The University reserves the right to, without prior notice, change or delete, supplement or otherwise amend any information,
requirements, and policies listed herein.
