• ASC Team who engage directly with
people with dementia and their carers
• Flexible short term interventions
• Carers’ assessments
• Ensure appropriate referrals within ASC
and to other organisations and feedback to
primary care referrers.
Carers Breaks Engagement Team
• Work with an individual to identify
services/activities in the community &/or
Personal Assistants
• Provides carer with a regular break
• Primary care referrals can be accepted for self
funders and patients who have declined a formal
social care/carers assessment
• Refer via HSCC (see next slide)

Health and Social Care Connect manages access and referral routes to
Adult Community Health and Social Care Services. It is jointly delivered by
ESCC Adult Social Care and East Sussex Healthcare NHS Trust (ESHT)
It is the conduit for health and social care professionals to access a range of
Adult Community Health and Social Care Services including: Joint
Community Rehabilitation, District Nursing and Intermediate Care Beds.
Health and Social Care Connect operates 7 days a week, from 8am to
10pm, including bank holidays.
If you need to make a referral please do this by completing a BICA and
email to esh-tr.icap@nhs.net or phone 0300 678 0010
If you are making a referral for JCR you can refer using a copy of your up to
date support plan
Health and Social Care Connect will then pass the referrals and any
relevant supporting information on to the required community service
Carers Breaks Engagement Team
• Possible interventions include:
 Set up urgent care packages
 Home visits
 Accompanying patient to GP or other health
appointment including Memory Assessment
 Assistance with managing medication
 Support to maintain caring role
Bexhill Pilot
• Dedicated resource for Federation of
Bexhill practices known as Integrated
Community Care (ICC)
• Patients with dementia
• Carers
• Single & short term interventions
• Direct liaison with the practices, attending
MDTs, visiting practices weekly
EHS and H&R Dementia
Support Workers
• 4 dedicated support workers to respond to
patients with dementia identified through
primary care to:
 Access services and community activities;
 Identify needs of carer
 Support patients to return home from respite
 Identify appropriate residential care
 Medication compliance
 Access to assistive technologies

• CRESS – emergency respite
• East Sussex Carers Card discounts
• Flexi TeleChecks
Respite for Healthcare
Appointments & Training
• Extra help or funding to look after cared for
person, so carers can take care of their
