Before being admitted to the program an applicant:
o must have a student/Empl ID number at LFCC
o must hold a high school diploma or GED (official copy of transcripts required)
o must hold a current unrestricted license as a practical nurse in the state of Virginia prior to the March
31, 2016 application deadline. Submit a copy of your current license with your application.
o must have completed high school biology, chemistry, and algebra with a grade of C or better or an entrylevel college course may be substituted in each subject (ie: CHM 101, 111; BIO 101; Mth 158)
o must have completed LFCC Placement testing if necessary. Must place into ENG 111 and show
competency in MTE 1- 6 or completed MTH 158. Documentation of proof to be included.
o must read and sign Computer Competency checklist – enclose with application
o BIO 141, 142 & 150 must have been completed within the past 8 years.
o must have taken the LPN to RN entrance exam administered in the LFCC Testing Center between
January 18, 2016 - March 30, 2016. Test must be scheduled with the Testing Center. Retakes will be
given in March only.
o GPA must be ≥ 2.5 by the end of Fall 2015, calculated on the last 12+ credits, to be eligible for the 2017
program
o must have completed the following with a grade of a “C” or better or be enrolled for the Spring 2016
semester

Include your Practical Nursing School. Unofficial transcripts from LFCC or other VCCS colleges are acceptable and
must be included. You are responsible for ensuring that the most current transcripts from any college are on file at
LFCC.
If you are transferring credits from another college (VCCS included), you must complete a Transcript Evaluation Form
and provide an additional official copy of your transcript if it is from a non VCCS college for evaluation by Admissions
and Records.

Applicants must have a complete application packet by the stated deadline to be considered. No exceptions.
The application packet consists of:
o a completed Advance Placement Option Application – Spring 2017
o official transcripts from high school or GED completion.
o official transcripts from all colleges and institutions attended (including Practical Nursing
School) and unofficial transcripts for any VCCS college attended
o Placement test results for the HESI LPN to RN entrance exam
o a copy of your current unrestricted license as a practical nurse in the state of Virginia
You will receive an email from the Middletown Nursing Department notifying you that your application is complete.
Please be patient, it may take several weeks.
