CONFERENCE PACKET ORDERS – CONFERENCE VOUCHER REFERRALS
INSTRUCTION
With a conference referral, each student will receive
a back-pack or book-bag filled with basic school supplies by grade-level
and a $15.00 gift card to Payless Shoes
(Shoe cards will be given to the parent or conference member)

CONFERENCE GROUP ORDERS – INSTRUCTION
Group orders are for Conferences that order back-packs for the families they
serve. It is the Conference responsibility to pick up their order and make their own
arrangements to deliver the backpacks to their families. (Many arrange
distribution from their own parish)
The Conference Group Order Form is posted on the website. Please fill out the
form in its entirety.
Mail or drop off the Conference Group Oder Form at the Council office or the Store
courtesy counter on or before August 7.
CONFERENCE VOUCHER FORM REFERRALS – INSTRUCTION
Store Voucher forms are used for families who will be picking up their own supplies
at St. William on August 13 or 14th.
The standard Store Voucher Form is used
***Do not include requests for other merchandise on the Voucher Back-to-School referral

Name of Conference
Conference member signature
Fill in family full name, address and phone number
List each child – children name, GENDER and grade level (Very important!)
Mail or drop off the original Back-to-School Voucher Form to the Waukesha Council office
or Store (818 W. Sunset Dr.) on or before August 7.
IMPORTANT! Give a copy of the Voucher form to the parent and instruct them to
present it at the time of pick up. This saves a tremendous amount of time the day of
distribution. Please note, if we do not have a record of the Conference Voucher referral,
the patron will not be able to receive a back pack. We will make note and if there are any
backpacks left over we will contact them.
A data base will be created listing all conference referrals. Only those who are referred
by a conference and submitted to the Council office by August 7th can be guaranteed to
receive school supplies, a backpack or book-bag and a shoe gift card. This is necessary due
to the time involved to purchase supplies not donated, the grade level packet preparation
and timely receipt of Payless shoe gift cards.
VOLUNTEER YOUR TIME August 3rd – 14th
 Monday, August 3 - help set up the gym at St. William.
 August 4 - 14 Volunteers are needed to help organize grade level supplies and
backpacks by gender and grade level.
 August 13 - 14 Volunteers are needed to help distribute backpacks and shoe gift
cards.
 August 14 (Afternoon) – Clean-up
 Choose and confirm your days/times you are able to help out!

August 13 – 2:00 pm – 6:00 pm
August 14 – 9:00 am – 1:00 pm
Advance Order - Conference Packet Group Order Pick-up (Conference member
picks up)
 As conference group orders are filled you will be notified. Group orders may be
ready prior to Aug 13 & 14th.
 If you will be picking up a conference order for multiple families, call ahead.
We will make arrangements to help load your vehicle.
Conference Voucher Referral Pick-up (Patron picks up)
Each parent or individual must present a copy of the voucher at the time of pick up.
Please help us! Students and parents are often waiting in line. A voucher in hand
greatly simplifies a swift, efficient distribution!
PAYLESS SHOE GIFT CARDS
 All conference packet orders and conference voucher referrals will
automatically receive a $15.00 payless gift card.
 Those requesting Payless Gift Cards only must place the order by August 7.
