Thank you for your decision to adopt a Georgia Peach. As you may have already learned,
adopting a Georgia Peach is a decision that will bring you utmost joy, but is not to be entered
into lightly. Dogs from Georgia Peaches Rescue have benefited from a high level of investment
and dedication that already saved them from a disadvantaged, neglected, or abusive life.
Georgia Peaches Puppy Rescue has created this adoption contract to ensure that these dogs
never see another day of abuse or neglect, and to protect the canine population, as a whole. If

Georgia Peaches Puppy Rescue (GPPR) agrees to provide a dog with testing for heartworm disease at six months,
vaccinations for rabies, dhpp and bordetella, de-worming, spaying/neutering, and micro-chipping as well as an
initial veterinary exam. The adopter will be informed of any known history of medical or behavioral issues. Each
Georgia Peach Puppy has been examined by a veterinarian in Georgia and may have also been seen by a second
veterinarian here in Seattle. Any repeat exam by your own vet leading to a new and previously undetected medical
issue must be identified within two (2) weeks of your execution of this contract. GPPR agrees to refund the
adoption fee if the adopted animal is returned unharmed within two weeks of the adoption date.
In exchange for the Adopter executing this Adoption Agreement and paying the adoption fee,
GPPR transfers ownership and possession of the above-identified dog to Adopter, subject to the
following agreed conditions:

If this dog does not adapt to my home, I cannot keep the dog for any reason, or if I wish to euthanize the
dog for any reason, I will contact GPPR and give GPPR the right of first refusal, meaning that GPPR may
take the animal back before I relinquish custody or possession of the animal to anyone else and especially

2. From the date I sign this contract, I agree to assume full financial and responsibility for the veterinary
care for this dog, including an annual examination and routine vaccinations and tests. I will keep the dog
free of pests parasites, ticks and fleas. If for any reason I cannot provide this care, I will notify GPPR

3. I understand that I am adopting a dog from a southern state where heartworm is prevalent. I understand
that if my dog is 6 months of age or older, (s)he received a Heartworm test prior to flying; if my puppy is
younger than 6 months of age, I understand (s)he will need to be tested for Heartworm at 6 months of
age. Due to the nature of Heartworm, GPPR requires a second testing 6 months after the puppy arrives in
Washington, and monthly preventative treatments after the date of the negative test result for the next 6
months the puppy is in Washington. I will ensure that my dog receives a heartworm test on

dog with monthly heartworm preventative to provide additional protection against heartworm until

4. I agree that I am taking this dog to be a member of my household and my family. I will be responsible for
providing a secure, indoor place to sleep and daily meals, as well as providing adequate supervision and
training to keep him/her safe. Training will not consist of force, fear, or pain but instead utilize a positive
approach. I will not train this dog to fight or be aggressive in any form. ___ Initials

I will provide this dog with a fully fenced-in yard, unless otherwise approved by GPPR. Electric fencing
should not be considered the sole barrier, and this dog will have additional supervision. This dog will be
kept on a leash at all times when outside of a securely fenced-in area. He/she shall not be kept chained or

6.
I will enroll my Georgia Peach puppy at an approved training facility for puppy play or appropriate
training class prior to or within 1 week of adoption and provide proof of completion upon request. ___

7.
I will provide a collar for the dog to wear at all times with the microchip tag and an identification tag
(providing the dog’s name, address, and phone number) affixed to the collar at all times. If I wish for a
rabies tag, I understand that I can request one through GPPR. ___ Initials
8. I agree to not permit the dog to ride in the back of a pickup truck unless secured in a carrier. ___ Initials
9. I will keep the dog’s microchip registration up to date, and will notify the microchip company of any
change of contact information. I will place Georgia Peaches as an alternative contact on the microchip
registration to ensure the dog’s security and safety in the event of emergency, or in a case I am not able or
willing to retrieve the dog.

11. I agree to not abuse or neglect the dog and under no circumstances will physically punish the dog. I
understand and agree that a citation for a civil infraction or criminal charge referencing any local, state, or
federal animal neglect or cruelty law (whether or not I am found guilty or to have committed the
violation) will constitute a breach of this agreement. If behavioral issues arise, I will consult with GPPR,
my veterinarian, or a trainer about the appropriate ways to deal with specific behavior problems.

12. I consent to the examination of this dog by GPPR at any time (with 48 hours’ notice) and acknowledge
that GPPR has the right to reclaim this dog at any time for failure to comply with the terms of this
contract or for any misrepresentation of fact made on the adoption application or in this contract.

13. I understand that the adoption fee is non-negotiable and non-refundable, except as provided in

15. I confirm that all information given in this contract is correct and accurate. I also understand that once
the contract is signed and I have taken possession of the animal, I agree to be solely responsible for any
and all harm, damage, or injury inflicted or caused by this dog, to any person, animal, or property,
including myself, my animals, and my property I agree to hold harmless, defend, and indemnify GPPR
and its representatives from and against any claims, demands, liabilities, injuries or losses caused by or
that arise from any act or omission of this animal or myself. ___ Initials
16. I understand that I have been informed of any known history of medical or behavioral issues. I further
agree to being solely responsible for any disease, deformity, illness, or health issue related to this dog and
adopt this dog as-is, with all faults, and without any warranties whatsoever PROVIDED THAT I may pay
for a veterinary examination and if that veterinarian finds a new and previously undetected medical
issue, which I then identify and disclose to GPPR within 14 calendar days of signing this adoption
contract, GPPR will refund the adoption fee upon my returning the dog to GPPR unharmed within 14
calendar days of signing this adoption contract. ___ Initials
BREACH – REPOSSESSION – FEES – CHOICE OF LAW - VENUE – JURISDICTION: I understand and agree
that GPPR has the right to demand immediate return of the dog for my breach of this Agreement and
that I not only am not entitled to a refund of the adoption fee, but that I must pay also all costs associated
with such repossession and restoration of the dog, including attorney’s fees and litigation-related
expenses. Further, I agree to pay additional attorney’s fees/costs related to collecting on any judgment
entered against me. Restoration includes grooming, veterinary care, resocialization, and other expenses
incurred to restore the dog to adoptable health. “Breach” is defined as a violation of any of the
conditions initialed by me in this Agreement. Any breach is agreed to be “material,” permitting GPPR to
rescind the Agreement and claim all remedies agreed to herein. Lastly, I agree that any dispute arising
from this Agreement will apply the laws of the State of Washington and be commenced in a court of the
State of Washington. I further agree that the court will have personal jurisdiction over me even if I
and/or the dog are no longer in the State of Washington. _____________ (initial(s))
ENTIRE AGREEMENT; MODIFICATION; BINDING EFFECT. This Agreement is the entire
agreement between me and GPPR and supersedes any prior understandings between us with respect to
this adoption. No modification of this Agreement will be valid unless in writing signed by me and
GPPR. This Agreement is binding on me and my successors, agents, heirs, assigns, executors, and

information provided on our Adoption Application is true; that I have read this Agreement, understand
it, have had the opportunity to consult with independent legal counsel, and agree to all conditions set
