Sample Mentor Agreement Agreement between Student and Mentor SDSU/UCSD Joint PhD Program in Global Health Date: Mentor (Name, Title, email, phone): Student (Name, email, phone): Expectations and Objectives This document represents an agreement reached between the graduate student and the faculty mentor/research advisor regarding the structure of the working relationship during the student’s tenure in the first two years of the SDSU/UCSD Joint Degree PhD Program in Global Health. It should be developed interactively between the two parties and modified according to mutual agreement on an annual basis. A copy of this agreement and any subsequent modifications should be dated and sent to the Program Coordinator at SDSU (rlopez@mail.sdsu.edu). The student should keep a copy of this agreement in his/her e-learning portfolio. Please use as much space as needed. 1. Regular one-on-one meetings. We plan to meet: (e.g. weekly on Wednesday, 1st and 3rd Thursday of the month, as needed but at least monthly, etc. During the first year, meetings should be monthly) 2. Participation in group meetings (if relevant). Mentor will invite student to participate in the following ongoing research or program group meetings (e.g., journal club, UCSD JDP Trainee Sessions, etc.): 3.
