FARE Guidelines for New Investigator and Mid-Career Awards Initial Funding Request Guidelines To apply to Food Allergy Research & Education, Inc. (“FARE”), a 501(c)(3) non-profit organization, for New Investigator or Mid-Career Awards, please submit a three- page concept proposal, along with an Initial Request Cover Sheet for Research Support (four pages total) plus the biographical information requested below. Please be certain to check the box for the type of grant you are applying: New Investigator or Mid-Career. Applications not following these guidelines will not be considered. On the first two pages, include the following: • Title of the project. • A brief (two-sentence) description of the project. • Overall objectives and significance of and benefit from your project. • Clearly defined specific goals of your project, how you will accomplish these goals, and the time frame of the project. FARE will primarily focus on the stated goals and the plan to accomplish them in reviewing all requests. • A top level breakdown of how the Award will be utilized (salaries, lab, supplies, overhead, etc.) Please note restrictions on percentage for laboratory and overhead expenses. For New Investigators, lab expenses are limited to $10K per annum. For Mid-Career, lab expenses are limited to $20K per annum. Overhead is limited to a maximum of 10%. On a separate single page (page three), please provide: • An explicit description of all related funding, active or pending. • A list of all submissions, or planned submissions, for grants to be funded by 501(c)3 organizations (other than FARE), foundations, or the US Government. Include a brief description of the goals, objectives and timing for each. • Any affiliations of the applicant. Biographical Information (form attached) • Biographical Sketch and Other Support for the Principal Investigator (P.I.) and for each CoInvestigator (if any) named on the Application Cover Sheet for Research Support. Limit four (4) pages per investigator. Electronic submissions are encouraged and should be sent to: research@foodallergy.org. Hard copy submissions will also be accepted and can be mailed to the address below. Investigators whose concept proposals are judged to be most meritorious and in line with FARE’s overall strategic vision will be invited to submit a more detailed follow-up application. Follow this format for each person. DO NOT EXCEED FOUR PAGES. DEGREE INSTITUTION AND LOCATION MM/YY FIELD OF STUDY (if applicable) NOTE: The Biographical Sketch may not exceed four pages. Follow the formats and instructions below. A. Personal Statement Briefly describe why your experience and qualifications make you particularly well-suited for your role. B. Positions and Honors List in chronological order previous positions, concluding with the present position. List any honors. C. Selected Peer-reviewed Publications The individual may choose to include selected publications (up to 15) based on recency, importance to the field, and/or relevance to the proposed research. D. Most relevant to the current application: Additional recent publications of importance to the field (in chronological order): Research Support List both selected ongoing and completed research projects for the past three years. Begin with the projects that are most relevant to the research proposed in the application. Briefly indicate the overall goals of the projects and responsibilities of the key person identified on the Biographical Sketch. Do not include number of person months or direct costs.
