The following is an actual savings account form
available online from the Emigrant Savings
Bank in New York. There are several kinds of
accounts available, such as regular savings
accounts, certificates of deposit, joint accounts,
and trust accounts. Suppose you want to open
a 6-month individual certificate of deposit by
yourself (not jointly or in trust for someone else).
(When you sign the form) you promise not to
withdraw the money for at least 6 months or
you will have to pay a penalty.

fact, thought, opinion, and humor. At the
newsstand it costs $2.00 per month, so 50%
can be saved on a year’s subscription. The
magazine’s actual address should appear at


Write a new order form for a weekly magazine,
which costs either $52.00 per year or $2.50 per
week. Assume that the magazine is Time
Magazine (you may be familiar with it). Make it
clear that the customer saves 60% by ordering
this magazine for a full year. One may pay by
sending a check or billing it to a credit card. If
they use a credit card, they should say whether
it's Visa, Master Card, or American Express.

about a person. Be sure to fill out all
sections in full.

Every student must register for classes
each semester. Registration includes
enrolling in classes, the telephone
registration system, paying fees and other
financial obligations, filing (提交…以备案)
your current address with the Office of the
Registrar (注册办公室), and providing
other information forms.
