Your health is important to you. It’s important to your family
and friends too.
Your health is also important to Uintah
School District. As your employer, we recognize the value of
investing in a Wellness Program which promotes healthy
lifestyles and builds a district wide culture of wellness.
For this reason, Uintah School District is offering you and
your spouse an opportunity to participate in the school
district’s Wellness Program and what we call the “Employee
Wellness Points Fund”.
The Wellness Points Fund offers a
variety of health and wellness activities in which you can
earn points for completing. When you earn enough points,
your portion of the cost for PEHP Medical Insurance
coverage will be reduced by 7%.
The information in this booklet will further explain how our
Wellness Program works and what you can do to participate!
Remember, what we do today to eat better, reduce stress,
or exercise more will affect how well we live tomorrow.

How to participate…
If you are enrolled in PEHP Medical Insurance
Employees are required to earn a minimum of 800 points during the current
school year in order to be eligible for the 7% insurance premium reduction for the
following school year. Participation by spouses is on a voluntary basis. Those spouses
who do take part may contribute up to 100 of the 800 points necessary! Employees
and spouses electing to participate in the Wellness Program must submit a signed
Name Release form to Cathy Bilbey at the district office.
How to get started
Review the Employee Wellness Points Fund Calendar to find your points earning
choices. Select the activities in which you want to participate. Each activity you
complete earns you the points associated with that activity. The Employee Wellness
Points Fund opens September 1, 2015. The deadline to earn your 800 wellness points
for a 7% premium reduction for the 2017 school year is August 31, 2016.
All preventive screenings, Wellness My Way, and Wellness Champion/Fit Choices
require signed Uintah School District Wellness Verification forms. Submit completed
forms to Cathy Bilbey at the District Office. Verification forms are not necessary for
any PEHP Healthy Utah wellness activity, i.e. cholesterol management, webinars, etc.
PEHP Healthy Utah will send verification to Uintah School District when you sign up
online.
How points are tracked
The points you earn between September 1, 2015 and August 31, 2016 can be tracked
by going to the Uintah School District website at www.uintah.net and clicking on
“Employee Wellness” and “Track Wellness Points”. Points are updated monthly. Look
for your employee number (sorted numerically) to see the activities you have
completed and the points earned.

GOAL: Your journey to wellness begins with a free yearly biometric
screening. Screenings are available by online appointment only
during the months of August, April, and June for Uintah School District
employees and spouses. The feedback you get from this assessment
can be the starting point for improving your health.

Healthy Utah at no cost to me; however, I understand that I may choose to go to my
personal physician for this yearly screening.

physician, PEHP Healthy Utah, Uintah School District or any other person or entity involved

Participate in the PEHP Healthy Utah Waist Aweigh weight
management program. To begin the Waist Aweigh program, go
online to www.pehp.org. To qualify for Waist Aweigh your BMI must
be 30 or higher. Waist Aweigh will help you succeed in reaching your
weight and health goals. PEHP coaches will work with you in a
confidential partnership via phone to get you started, monitor your
progress, and celebrate your success.
To earn your 250 points, no proof of verification is necessary. PEHP
Healthy Utah will send verification to Uintah School District when you
sign up online.

TOBACCO CESSATION
Worth 150 points
GOAL: Quit smoking by enrolling in the PEHP Healthy Utah Tobacco
Cessation Program.
To participate in the PEHP Healthy Utah Tobacco Cessation program,
go online to https://pehp.quitlogix.org. It is difficult to quit smoking,
but the PEHP Quitline is available to help! Quitline is a free program
that provides helpful tools, information and personal coaching
throughout your quit process.
Participate in 5 Quitline calls and remain tobacco free for 6 months.
To earn your 150 points, no proof of verification is necessary. PEHP
Healthy Utah will send verification to Uintah School District Wellness
when you sign up online.

BLOOD PRESSURE
MANAGEMENT
Worth 100 points
GOAL: Lower your blood pressure to within normal range (120/80) or
(140/90 – 150/90) for those over age 60. Physicians may verify blood
pressures within normal range per individual.
Participate in the PEHP Healthy Utah Blood Pressure Rebate program
by clicking on www.pehp.org. Choose “Rebate Program” and
complete the blood pressure rebate improvement.
To earn your 100 points, no proof of verification is necessary. PEHP
Healthy Utah will send verification to Uintah School District Wellness

GOAL: Help postpartum mothers to reach pre-pregnancy weight and
obtain educational resources.
To participate in the PEHP WeeCare and/or Postpartum Weight

you must be enrolled in WeeCare during your pregnancy. In addition,
you may earn a second rebate for postpartum weight loss.
To earn your 50 points, for either or both programs, proof of

CHALLENGES
Each Challenge is Worth 100 Points
GOAL: Participate in a six week Uintah School District Wellness
Challenge to increase awareness and improve health.
Four Wellness Challenges will be offered throughout the school year.
You may complete one or all four of the challenges. Registration for
the challenges will be sent out via email to all employees one week
prior to the start date.
To earn your 100 points, return your tracking calendar to your
Wellness Representative at the end of the challenge.

WELLNESS MY WAY
Worth 300 points
GOAL: Capitalize on your personal interests and your preferred
method of staying active and healthy.
Wellness My Way is a self-directed approach to accomplish a personal
physical health and wellness objective. Choose a physical wellness
goal of your choice and work towards achieving it for a minimum of
three months. Map out your method to meet your goal. Examples:
Train and participate in an event such as a 10k Marathon or Triathlon,
Team Sports, Cross Fit, Fitness Classes, Swimming, Biking, Walking,
etc.
This health management objective can be counted twice per year.
To earn your 300 points, you must submit a signed Uintah School
District Verification form stating that for a minimum of three months
you participated in a Wellness My Way activity along with a
completed exercise log. Any exercise log is acceptable.*

GOAL: Be more physically active by joining your Wellness Champion
and coworkers in Fit Choice activities at your work location.
Participate, along with other coworkers, in a worksite Wellness
Champion sponsored activity before/after school. Activities may
include a team sport, aerobics, biking, walking, running, etc.
To earn your 150 points, you must submit a signed Uintah School
District Verification form stating that for a minimum of six weeks, you
participated with your Wellness Champion in a Fit Choice.*
This health management objective may be completed up to four times
per year.

Wellness Program Q & A
1. How does the Uintah School District Wellness Program benefit me?
By electing to participate in the school district’s Wellness Program you will receive a reduction in
the cost of your medical insurance premium. For the current school year, this is a yearly savings
of $1,176.00 for someone on a PEHP Preferred Family plan! Not only will you see monetary
benefits for participating, but also the benefit of having fun with co-workers and at the same
time, hopefully, improved personal health and wellbeing!
2. I’m a new employee to the school district. How do I get the insurance premium discount?
All new hires who qualify for health insurance automatically get the premium discount when
hired. To qualify for the discount at the start of the upcoming contract year, you will need to
complete the following steps during the current contract year and every year thereafter:
1. Submit a signed Name Release Form to Cathy Bilbey at the district office (one-time
requirement).
2. Earn a minimum of 800 points during the current school year in order to be eligible for
the insurance premium reduction for the following school year. If hired between
October 1st and November 30th you are required to earn 500 points. If hired after
December 1st, no points are required, however, you must complete any remaining
District sponsored Wellness Challenges.
3. Complete a PEHP Healthy Utah Health Screening before August 31st.
Or
Complete a doctor performed health screening before August 31st.
4. Complete the online, PEHP Healthy Utah Health Risk Assessment before August 31st.
Example: Sign a Name Release Form and submit it to Cathy Bilbey at the district office. Review
the Employee Points Fund Chart to find your points earning choices. Earn 800 points by
completing the activities in which you wish to participate. PEHP Healthy Utah Screenings will be
held in April, June, and again in August. Sign up online and complete 1 of the screenings. One
to two days after your screening, complete the online PEHP Healthy Utah Health Risk
Assessment. You now qualify for the health insurance premium discount for the upcoming
contract year.
3. Do spouses need to participate in the school district’s Wellness Program to receive the
premium discount, or just the employee?
Participation by spouses is voluntary. Only employees need to participate, although we
encourage spouses to join in. Spouses who take part in both the Biometric Screening and the
Health Risk Assessment can contribute 100 of the 800 points necessary for your premium
reduction. All spouses who choose to participate must sign a Name Release Form.
4. How often do I need to have a health screening and complete the PEHP Healthy Utah Health
Risk Assessment?
To receive the premium discount, one screening per year is required. Example: If you were
screened in April, your next screening would be done in April of the following year. A screening
year runs from September 1st to August 31st. Once you’ve completed your annual screening, you
will need to complete the PEHP Healthy Utah Health Risk Assessment by going to PEHP’s
website at www.pehp.org. Instructions for completing a Health Risk Assessment are available
on the school district website under “Employee Wellness”.
5. How will I receive credit for having a PEHP Healthy Utah Health Screening and completing the
PEHP Healthy Utah Health Assessment?
All employees electing to participate in the school district’s Wellness Program will be required

Health Risk Assessment. All health information is private and only a list of names will be
provided. If your name is on the list, you will receive the credit!
Where do I get a Name Release Form and where should I submit the form?
Release forms are available on the school district website under “Employee Wellness”. Once
you have signed the form, submit it to Cathy Bilbey at the district office. Forms will only need to
be signed one time to participate in the Wellness Program.
How do I arrange to have a PEHP Healthy Utah Health Screening?
The district arranges for PEHP Healthy Utah to provide health screenings for PEHP insured
employees three times yearly, April, June and August. To schedule an appointment, log on to
PEHP’s website by going to www.pehp.org or call them directly at (801) 366-7300. Step by step
instructions for scheduling a testing session are available on the school district’s website under

I prefer having a health screening performed by my doctor. What do I need to submit to PEHP
Healthy Utah so that I can complete the PEHP Healthy Utah Health Risk Assessment?
First you will need to sign a copy of the Waiver of Biometric Screening by PEHP Form and return
it to Cathy Bilbey at the district office. Forms are available on the school district website under
“Employee Wellness”. Next, take the PEHP Healthy Utah First Steps & Good for You Form with
you to your doctor’s appointment. Copies of the form are available on the district website
under “Employee Wellness”. Your physician will need to perform the standard biometric tests
on you, i.e. BMI, Height, Weight, Cholesterol, etc. and sign the First Steps & Good for You Form.
Submit the form, along with your lab results, to PEHP Healthy Utah by fax to 801-328-7300 or
mail to PEHP Healthy Utah, 560 East 200 South, Salt Lake City, UT 84102. Allow approximately 2
weeks for PEHP Healthy Utah to enter your information in their system. After the 2 weeks are
up, log on to PEHP’s website by going to www.pehp.org and complete the Health Risk
Assessment. Instructions for completing the assessment are available on the school district’s
website under “Employee Wellness”.
I had a PEHP Healthy Utah Health Screening this past April, completed the PEHP Healthy Utah
Risk Assessment and earned 800 points. Will I get this year’s premium discount?
Yes, as long as the district has record that you completed both the PEHP Healthy Utah Health
Screening and Health Risk Assessment, and submitted all required documentation and earned
the necessary 800 wellness points by the August 31st deadline.
I am on a district insurance plan that has no premium cost to me. Should I still participate in
the Wellness Program?
Yes. If insurance rates go up in the future, there may be a premium cost for you, the employee,
attached to the plan you are currently on. If you have not completed all of the required
elements of the Wellness Program during a given school year, you will not receive a possible
future reduction in your medical insurance premium.
I chose to waive the district medical insurance. Should I participate in the Wellness Program
even though there is no insurance cost to me?
It is recommended that you do so. In the future, if you elect to take the district’s medical
insurance and you have not earned 800 points during the prior school year and provide proof of
a screening performed by your doctor, you will not receive the reduction in your medical
insurance premium. Participation in the PEHP Healthy Utah Health Risk Assessment is not
required for those who waive their insurance.
