How to register If you have a medical card, check your address on it is up to date. Then, sign and date the card and bring it in to the Practice. Please note that unfortunately, we do not register patients who live outside our practice boundary. If you do not have a medical card you will be given a GMS1 form to complete. You will need to provide: the name and address of your last doctor your home address when you were registered there proof of your current address photo ID such as your passport if you are from overseas, your place of birth and the date you arrived in the UK If you require on-line access you will need to complete the practice terms and conditions forms All new patients will be given a health screening form to complete. These documents need to be completed and returned to us before we can process your registration. Carers If you are a Carer for a family member or friend please let our Reception team know so that we can make sure we have this on your records. The Alexandra Practice Access to records The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances: To provide further medical treatment for you e.g. from district nurses and hospital services To help you get other services e.g. from the social work department. As a patient registered at the Practice, you can choose to see any of our doctors. If you require same day attention, please call between 8.30 am and 10.30am on the day and we will arrange a telephone consultation for you with the on call doctor, or an appointment with the Practice Nurse. Face to face assessment will be arranged as needed. If you’re not sure which member of our healthcare team you need to see, the reception team may be able to help. Office Staff Our Practice Manager, Mrs. Helen Brown, is available to listen to any comments you have, good or bad, about the Practice. Please address any complaints to her in writing in the first instance. Audrey is our secretary, and Shirley, our Assistant Practice Manager, leads our reception team. When you come in or telephone you are likely so speak to one of our administrators: Alison, Sarah, Rehana, Kim or Jo who will try to help you with your query. We also have a dedicated receptionist, Beverley who works Monday; Tuesday; Thursday and Friday. Other people you might see We work closely with other professionals who deliver services from the Practice. These include midwives; counsellors; the alcohol team and a psychologist; district nurses; active case managers and social workers. We also run a private Travel Health Clinic for services not available on the NHS. Why not ask at reception for more details. Home Visits If you are too ill to attend surgery, please try to ring before 10am to inform us. Urgent calls can be made at any time. We need your contact telephone number so that the doctor can ring you to assess your problem quickly. Prescriptions Prescriptions for regular medications can be ordered from the surgery, but please give 48 hours notice, so do not wait until you have run out of medication before organising these. Emergencies Out of Hours Please telephone the normal surgery number outside normal surgery hours. You will be transferred to the Out of Hours service. They provide cover when our surgery is closed. NHS Direct can be contacted on 111. Repeat prescriptions can be ordered in person at reception, by fax, by post or online via our website. Unfortunately we can only accept telephone requests for housebound patients. You can arrange for a pharmacy to collect your prescription from us and deliver the items to you. Please contact the pharmacy directly to arrange this. Training Practice As a training practice, we have some doctors with us for periods of up to a year. These are fully qualified doctors who wish to enter general practice. Training may involve video recording of consultations, with the full consent of the patient. This is always confidential. Sometimes we also have medical students in the Practice, sitting in with other doctors. Practice Area We can only accept new patients if they live in our practice area. Nursing Staff Claire Williams is our Practice Nurse and is available for cervical smears, childhood vaccinations, and screening and monitoring of long term conditions such as asthma and diabetes. Additionally, Claire can see and treat minor illnesses and injuries. Jane Walton is our Travel Clinic nurse and will administer travel vaccinations. We have a disabled toilet on the ground floor, and a stair lift for accessing consulting rooms upstairs. We also try to accommodate specific requests to be seen in a downstairs consulting room.
