Birmingham-Southern Music Department Recital Class Form IMPORTANT: read the following guidelines to receive approval for a Recital Class Performance: Only BSC Faculty members can submit forms for student recital class performance. This Recital Class Form should be completed by the student and emailed to the private instructor as a word document for instructor approval. Upon approval by the instructor, the instructor should email the Recital Class Form as an attachment in a word document format to Mr. Posey at bposey@bsc.edu. The Recital Class Form must be submitted before 1:00 PM on the Monday prior to your requested Tuesday Recital Class Performance date. Incomplete forms, late forms, or forms sent by students without a supporting email from the private instructor will not be accepted. 1. Voice Classification or Instrument 4. Name of Accompanist Please note if the accompanist will be performing on Piano or Organ. If the work does not require accompaniment, leave this section blank. 5. Title of Work If you are performing a movement from a multi-movement work, include the title of the work and the title(s) of all the movement(s) that will be performed. Example: Sonata in D, Op. 1, No. 2 I. Andante III. Allegro If you are performing a single piece of music from a larger work, include both, the name of the single piece of music being performed and the title from the larger work. Example: Thou Art Gone Up on High from Messiah 6. Name of Composer and his or her dates 7. List all equipment (including piano, the number of stands, chairs, etc.) that is needed for your performance
