1040 NR(earned wages in another state) Your state taxes will require you to complete two forms: the Illinois 1040 and the Schedule NR. Please follow the following instructions. We are unable to help you file the tax forms from any other states you worked in. Part 1: Begin with your 2012 Form IL-1040 form Step 1: Fill in the necessary personal information. Steps 2 and 3: Find your Adjusted Gross Income as reported in Line 10 of your completed federal 1040 NR-EZ form. Enter that amount in Lines 1, 4, and 9 of your Illinois 1040. Step 4: On Line 10a enter 1 x 2050 a 2050. Then at the bottom of that section, on the Exemption Allowance line, enter 2050. Part 2: Now switch to your 2012 Schedule NR form Step1: Check "NO" because you are not a resident. Step 2: (You've already done that above) Step 3, 4 and 5: If your 2012 income was earned in Illinois AND at least one other state, take the number you entered in Line 3 of your federal 1040 NR-EZ and enter it in the following places: -Line 5, Column A -Line 37 -Line 47 Take the amount of money you earned in Illinois (from Box 1 of your Illinois W2 forms) and enter that amount in the following places: -Line 5, Column B -Lines 20, 21, 38, 41, and 46 Then, enter the number "0" in the following places: -Lines 36 -Line 39 columns A and B -Line 45 Line 48: Divide Line 46 by Line 47 (carry to three decimal points) and enter that amount here. Lines 49: enter "2050" Line 50: Multiply Line 48 by Line 49 and enter that amount Line 51: Subtract Line 50 from Line 46 and enter that number Line 52: Multiply Line 51 x .05 and enter that number Part 3: Now return to your Illinois 1040 document Step 5: Line 12: Check the Nonresident box and enter the amount from Line 46 of your Schedule NR here. Steps 6 and 7: Lines 13, 15, 20, and 21: Enter the amount from your Schedule NR Line 52 Step 8: If you have purchased anything online or outside of Illinois that you did not pay sales tax on, enter 3 on Line 23. If you have not purchased anything online or out of state, enter 0 on Line 23. Then add the amounts in Lines 21 and 23 together, and enter that amount in Line 24. Step 9: Look at the bottom of your W-2 form(s). Enter the amount listed in the "State Tax" box in lines 25 and 29. Again, if you have more than one W-2 form, total the state tax from all W-2s and enter the sum in Lines 25 and 29. Step 10: -If the amount you entered on Line 24 is less than the number in Line 29, you have overpaid and the state government will refund the difference to you. Write the amount you overpaid (the difference between Line 29 – Line 24) in Lines 30, 35 and 36. If you would like your refund deposited into your bank account, enter your account information in Line 37. If you want a check mailed to you, leave Line 37 blank, and it will be mailed to your address given on Page 1. Then print 2 copies of the form, sign, and date them. Staple one copy of your W-2 form(s) to the document and put in envelope. Include one copy of your Federal 1040 NR-EZ form, and mail it all to: Illinois Department of Revenue PO Box 1040 Galesburg, IL 61402-1040 -If the amount in Line 24 is greater than the number in Line 29, you owe the state the difference. Subtract Line 29 from Line 24 and write that amount in Lines 31 and 39. Then print 2 copies of the form, sign, and date the form. Staple one copy of your W-2 form(s) to the document and put in envelope.
