Instructions:
This form is to be completed at a church board meeting that is called by the first elder. The board is to make every effort to
conduct the evaluation candidly and objectively. If board members are unable to reach consensus regarding the response to any
question, the rating that commands the highest number of votes is to be accepted.
Kindly mail or deliver the evaluation form directly to the Ministerial Secretary of your Conference.
