• All entries on the forms will be printed or typed except personal
signatures, initials, and stamps. All logbook and maintenance form
and record entries may be made in blue or black ballpoint pen, black
lead pencil, stamp, or they will be Unit Level Logistics SystemAviation (ULLS-A) or Logbook Automated System (LAS) generated
unless the specific instructions state otherwise.
• The minimum requirement to certify entries on all forms governed by
this pamphlet is the Personnel Identifier (PID).

• Calendar date format, for most of the forms and records in this
pamphlet, contains seven alpha and numerical characters plus two
spaces in the order of day, month, year (dd mmm yy). For example, 3
Oct 91 will be recorded as 03 Oct 91.
• Clock time format will be entered using the 24-hour clock. For example,
4 minutes after 9 o’clock in the morning will be entered as 0904, or 10
minutes after 1 o’clock in the afternoon will be entered as 1310.
• Forms and records must be readable, correct, and complete. If a form
is found to have missing or incorrect information, correct the form per

(1) Rated crew members.
(a) PI—pilot.
(b) CP—copilot. When briefed, this symbol may be used by more than one aviator
performing copilot duties.
(c) PC—pilot in command. The symbols PC, MP, ME, XP, UT, IE, IP, or SP may be
used to designate the pilot in command. If any of these pilot in command symbols are
mixed or duplicated on the same aircraft, the mission brief sheet must clearly indicate
which aviator is the pilot in command.
(d) MP—maintenance test pilot. This symbol may be used by both aviators, if
qualified, on functional test flights when authorized by the mission brief sheet.
(e) XP—experimental test pilot. This symbol may be used by both aviators on
experimental test flights when assigned to a designated testing organization or
activity and authorized by the mission brief sheet.

(2) Nonrated crew members use the following symbols to record flight time when
qualified and designated on the mission brief sheet to perform the duties specified by
the symbol.
(a) OR—nonrated aircraft maintenance personnel, technical observer, firefighter,
aerial photographer, door gunners, crew member training course students, crash
rescue specialists and other nonrated crew duties as approved on the mission brief
sheet.
(b) CE—crew chief, aircraft mechanic and non-crewmembers designated by the
commander and in the unit’s ATP.

the crew chief according to figure 2–7, and
the entries will be checked by the pilot in
command.

grade of fuel and oil, and the results of the in-tank checks.
The crew chief or mechanic will enter their PID in the

STATUS.
Enter the proper status symbol in this block. Once a status symbol is entered in the
STATUS Block, it will not be erased or changed even if entered in error (para 1–8).
The person who corrects a red "dash" or red diagonal "/" fault will enter their last
name initial over the status symbol. A red "X" or circled red "X" status symbol
will be initialed over by a qualified designated representative appointed by the unit
or activity commander. When clearing a red "X" or circled red "X" a ballpoint pen
with blue or black ink, or technical inspector’s stamp may be used. The
maintenance test pilot will initial the symbol after they complete the test flight and
the aircraft is cleared for flight. When more than one person completes the
corrective action for a fault, other than a phase, periodic, or progressive phase
maintenance, the senior mechanic, who performed part of the corrective action will
initial the status symbol for entries with a red diagonal "/" or red dash "-" status.
