WHAT IS EPC WELLNESS?
EPC Wellness will provide participants with health and wellness education. Those
enrolled in the health plan are eligible to participate.
Earn $100 Wellness Dollars
Step 1
Completion of the Personal Health Assessment (PHA) is required.
Step 2
Completion of Your Roadmap for Health Wellness Workshop
Step 3
Completion of a second Wellness Workshop (3 week minimum)
Step 4
Redeem wellness dollars in the Hallmark Gift Card Center
STEP 1: PERSONAL HEALTH ASSESSMENT (PHA)
A Personal Health Assessment (PHA) is a 15-20 minute questionnaire that asks about lifestyle habits,
such as diet, exercise, stress level or smoking habits.
After the PHA is completed, participants will receive a detailed report that rates their health risks (Low
Risk, Moderate Risk, or High Risk), as well as an overall wellness score.
The PHA will provide personalized recommendations about how to better manage their health based on
their risks.
The PHA is required, but participants will not earn wellness dollars for completion.
STEP 2: ROADMAP FOR HEALTH WELLNESS WORKSHOP
A wellness workshop is an online program that provides self-paced education around a
specific topic.
Wellness workshops include informative articles, tip sheets, wellness tools, and tasks.
These resources will help to empower the participant to make healthy behavior
changes.
Complete Your Roadmap for Health Wellness Workshop: $50 wellness dollars
4-week wellness workshop that reviews 4 key topics for achieving optimal health:
1. Awareness
2. Nutrition
3. Activity
4. Stress reduction

STEP 3: WELLNESS WORKSHOP
Step 3: Complete one additional wellness workshop (3+ weeks): $50 wellness dollars

STEP 4: REDEEM WELLNESS DOLLARS
Wellness dollars are paid out in the form of gift cards that can be redeemed at
www.EPCwellness.org in the Hallmark gift card center.
Participants can select from over 200 gift cards in the Hallmark gift card center.
Gift cards can be redeemed as one $100 or as two $50
gift cards
DISTRICT INCENTIVE
EPC school districts are eligible for the district incentive based on Your
Roadmap for Health Wellness Workshop completion rate

IMPORTANT DATES
October 5, 2015: Program Launch
April 1, 2016: Eligibility cut off date for new
hires or those new to the health plan. Those
hired or enrolled in benefits after April 1 will
be eligible for the next program year.
June 1, 2016: Last day to complete Your
Roadmap for Health Wellness Workshop to
meet district incentives
August 31, 2016: Last day to complete Your
Roadmap for Health and an additional
Wellness Workshop to qualify for wellness
dollars
September 30, 2016: Last day to redeem
wellness dollars for gift cards
CONFIDENTIALITY AND PRIVACY
Rite for You Corporate Wellness complies with all HIPAA Privacy and Security Standards, and maintains
the confidentiality of all information relating to employees who choose to participate – which means
that individual results are never released unless the appropriate written consent is provided by the
EPC employee. A high-level summary of the aggregate results will be provided to the EPC Human
Resources Department to better understand the impact their efforts are making on the overall health
and well-being of the EPC employees.
