Deliver LAB-R and Spanish LAB Answer Documents to Borough Assessment Office
All schools / Deadline: October 1
LAB-R and Spanish LAB exams are administered only when a student first enrolls in the
New York State public school system. Please hand-score the tests at the school site
and hand-deliver answer documents to your Borough Assessment Office by noon on
October 1. Assessment Memo #2. If you have additional questions, contact Jose
Garcia at jgarcia17@schools.nyc.gov.

LAB-R AND SPANISH LAB PACKING PROCEDURES:
1) Include principal’s certification (Attachment 4) (DOE username and password login required)
2) Package answer documents in four separate brown testing envelopes. Label each of the
four testing envelopes with one of the four labels below. Use a maximum of 4 brown
document return envelopes:
Envelope labels:
- LAB-R for General Education Students
- LAB-R for Special Education Students
- Spanish LAB for General Education Students
- Spanish Lab for Special Education Students
3) Poly bags are included in the packing materials received by the school. Place one gradelevel in each poly bag. DO NOT MIX GRADES IN A POLY BAG. Please fill out the header
on each poly bag according to these instructions:
GENERAL EDUCATION HEADERS
- fill in all areas on general education header
- In the teacher’s name field please write the grade level (i.e. teacher’s name: Grade 3)
- Please write 000 in the class field
SPECIAL EDUCATION HEADERS
- fill in all areas on special education header
- In the teacher’s name field please write SPECIAL EDUCATION
- Please write 000 in the class field
4) ANSWER DOCUMENTS
- fill in the following areas on bio side of answer document for ALL students – last name,
first name, student id #, date of birth, sex, GRADE, home language, FORM B, borough,
district, school, test date
- fill in the following additional information on the bio side of the answer document for
Special Education students, such as tested with modification (if applicable)
Updates on Periodic Assessments
Events/Deadlines: through October 13
Visit the Periodic Assessment team’s Web site for the latest updates on Periodic Assessments
and links to free professional development opportunities. Here’s what’s important this week:
ELL
ELL Fall Administration materials will be arriving in the schools that selected them by
September 28; please see the ELL Fall Administration (September 29-October 13) memo for
detailed information about this administration.
Teacher’s College Reading and Writing Project (TCRWP)
Schools that use the TCRWP curriculum and assessments but did not select TCRWP as
Periodic Assessment can complete this survey by October 1 to have their data loaded into
AssessmentPro. Schools that fill out the survey will see their student data in the system on
October 26. This opportunity only applies to schools that are already using TCRWP and will not
be available after the survey deadline passes.
Schools that signed up for TCRWP during the Periodic Assessment selections window last June
will have all data for students in grades K-8 loaded to AssessmentPro during the next data
refresh and will be available October 26.
