Part 1 Script: A 38-year-old female, Katherine Rivera, presented complaining of sadness and decreased energy. She
describes loss of interest in eating, social activities, sex and even talking with friends. She states that she finds herself
crying for no apparent reason and doesn’t really enjoy doing much of anything. She can stay in bed for the whole day
and has lost 11 pounds for the last 2 months. She has difficulty concentrating and can’t remember where she put
anything. She has never felt like this before and feels scared. She appeared cheerless, distressed and distracted and
spoke in a soft, low voice with little enthusiasm. To rule out any physical cause of her symptoms, a medical history was
taken and a PE was performed. A physical cause of depression was ruled out. She was prescribed Asendin, 25 mg twice a
day for six months and referred to see a counselor. A follow up visit was scheduled in six weeks.
