6th Grade Physical Education Sportfolio
This project must be in a folder before turned into a teacher.
Contents: Must be in order when turned in!!
1. Front Cover (10 pts) 5 points per 9 weeks.
a. Create a front cover for the front of your 3 ring binder.
b. Please include your Name (first and last), Class Period, and Grade.
c. Creativity may lead to earning extra credit.
2. Fitness Goal Setting Project (50 pts) 25 pts per 9 weeks
3. W.O.W. Words of the week (5 points per week) up to 60 pts for semester
a. Students will receive a word of the week. They must define the word and write a sentence on how that word
relates to physical education
b. Students will be turning these in every 9 weeks.
4. Fitness Terminology (100pts) 50 pts per 9 weeks
a. Using the outline provided. First 5 will be
b. Define the term in relation to physical education
c. Use a picture to represent the term. You can draw, use clipart, or a magazine picture.
5. Physical Activity Log (40 pts) 20 pts per nine weeks
a. Must have at least 20 days of physical activity outside of the physical education class.
b. At least 20 minutes total for the activity.
Sportfolio grades will be split into two nine weeks.

“W.O.W.”- Word of the Week
In order to increase and better understand your Physical Education, Health and Fitness
vocabulary, you will be given a Word(s) of the Week (W.O.W.), It will be your responsibility to
complete this template each week. At the end of the of the semester, you will be given a WOW
quiz to test how well you are learning key concepts of these words. Most weeks, I will check your
Sportfolio to see if they are there.
Your WOW template will be worth 60 pts. Five points per week.
