Prescription Refill Protocol

There are many legal, medical and ethical issues that must be considered when a medication is prescribed and a medication refill is authorized.

Please plan refills and appointments according to our policy.

1. We will refill medications prescribed by Kristie only . We will not authorize refills for medications prescribed by any other providers. Kristie is a wellness, age management, and hormone specialist. Refills for hypertension, diabetes, arthritis, headache, seizures, pain management, controlled substances, depression, anxiety, insomnia, cold, flu, allergy and other family practice, internal medicine or disease medications, etc. will not be managed by Kristie. Kristie recommends that you continue your relationship with your primary care provider for all chronic or emergent prescription and medical needs.

2. We refill prescriptions only when your office visits and screening tests are current. If you have not followed Kristie with appointments and screening as directed, prescriptions will not be refilled. Because Kristie monitors progress and response to medications on a regular basis for optimization, timely health screenings and appointments are required as directed by Kristie. Blood work must be performed, and appointments must be kept as directed..

3. You are responsible to provide our office with a copy of your annual health screening reports.

For women , this includes preventative gynecologic exam report, pap smear report, and bloodwork. Mammography, dexa scan, and colonoscopy will be required based on age. For men, this includes initial prostate exam and report, colonoscopy, PSA blood test, and other appropriate blood work. If current preventative reports, blood work, etc are not on file, prescriptions will not be refilled.

4. It is your responsibility to notify the pharmacy that you are in need of a refill prior to running out. The pharmacy will fax a refill request to our

office, and the necessary steps will be taken to refill your prescription. You must request all refills directly from the pharmacy. Please do not call our office for refills.

5. Plan your refills. Do not wait until you have one pill left to notify your pharmacy that you need medication refills . The pharmacy can fax a refill request as soon as you have filled your final refill. Some medications will take the pharmacy 2 days to prepare. Kristie may not be available to refill your prescription immediately. Plan at least 2 weeks ahead for your prescription refill needs to avoid frustration.

6. Allow 2 business days for prescription refill request to be authorized by Kristie. Please plan your refill requests accordingly. We recommend that you check with your pharmacy to determine if the prescription request has been authorized by our office before leaving your home to pick up your prescription. Please plan your appointments so that your prescriptions can be refilled in a timely manner.

7. Prescriptions will be filled during regular office hours Tuesday through Thursday 9a-3p.

8. We are unable to refill prescriptions on weekends or evenings when the office is closed. The office is closed on all holidays and weekends. The office may be closed at different times throughout the calendar year. The office will not refill prescriptions by phone.

9. As of August 1, 2103, We are unable to refill prescriptions to multiple pharmacies. In the past, Kristie has allowed patients to use multiple pharmacies for their prescription needs. This will no longer be the case. To avoid confusion and to integrate new prescription management software, the office will refill patient prescriptions to only one pharmacy of their choice.

10. The quality of your compounded-prescription medications is of the utmost importance.

Kristie is particular about the quality of your compounded hormones. If insurance will pay for compounded prescriptions,

Smith’s Rexall Pharmacy (801-785-3221) in Pleasant Grove and Rock Canyon Pharmacy in Provo (801-377-2002) are great options.

Medquest Pharmacy (888-222-2956) in Salt Lake City is Kristie’s preferred self-pay option. Smith’s Rexall and Rock Canyon also provide self-pay as well as insurance pay options.

All three pharmacies provide excellent quality, optimal hormone levels, and are cost appropriate. All will provide shipping.
