Survey Plan template

A survey plan shows what will be surveyed or inspected and when. Developing a survey
plan for each vessel in your operation is a requirement under Maritime Rule Part 19 .
This template will help you to develop a survey plan in a form that is acceptable to the Director of
Maritime New Zealand.
You need to include your survey plan (or plans) when you send your application for a Maritime
Transport Operator Certificate (MTOC) to Maritime New Zealand. The survey plan is part of your MOSS
(Maritime Operator Safety System) Operator Plan.
About this template
You need one survey plan for each vessel in your operation.
This template has seven sections:

3. Certificates and exemptions
List all required ship certificates and any exemptions granted by the Director of Maritime
New Zealand. Tick the ones that apply from those listed and add others as is appropriate.
Remember to include the certificate numbers and expiry dates. The certificate number, provided by the
certificate issuer, will be either a number or some other unique identifier. The expiry dates determine
the content of your survey items schedule – refer to 6: Survey items schedule.

6. Survey items schedule
This survey items schedule (see below) shows all of the items that need to be surveyed or
inspected and when this needs to happen.
This schedule starts after the initial survey is completed and runs for the full life of your vessel.
Remember to complete the calendar year and age of vessel information.
Use the table below as a guide to developing your own survey items schedule. Update this table with
the survey items that are appropriate for your vessel. Add more rows where you need them.
