Poverty - Uninsurance Cardiovascular - Cancer Child Health - Behavioral Health Other health indicators • Community data compared to national benchmark • Data availability varies by indicator (census tract, county, and state level data) Need for Assistance (NFA) Worksheets • Data sources and year are on worksheet for each indicator • NFA available for PCAs and counties – Will be posted on HSD website soon • Can also request NFA for your specific service area – Define service area by census tracts, zip codes, or street boundaries – Submit request to tracy.lenartz@azdhs.gov – Please allow 2 week turn-around time Need for Assistance (NFA) Worksheets Arizona Medically Underserved Areas (AzMUA) • Biannual designation process per ARS 36-2352 • Utilizes PCA boundaries • Designation is based on Primary Care Index, consisting of 14 weighted items: – – – – Provider Ratio Uninsured Births Infant Mortality Minorities -
