ISSUE 3 October 2015 Research Matters HSJ Award for Value and Improvement in the use of Diagnostics – MISSION Severe Asthma We are delighted to have won a prestigious national HSJ award with colleagues at the Wessex Academic Health Science Network (AHSN) for the Mission Severe Asthma project. The judges said: “Our winner has a fantastic multidisciplinary team approach. The impact is excellent, well evaluated and it works with high risk patients, which in the longer term will reduce death rates”. MISSION Severe Asthma recognises the benefits in supporting self-care and enabling patients to manage their own conditions. Launch of the R&I Patient Experience Survey September 2015 saw the launch of a survey to capture patient feedback on their research experience at every research visit. Both quick and simple to complete; it gives patients the opportunity to rate their research experience, say what was good and inform us what can be improved. The survey can be returned to staff or dropped into the departmental or main collection box situated near the health information desk (main entrance). A comprehensive, carousel-style system was used, including both clinical assessments and novel diagnostic tests, to characterise the patient’s asthma. Each patient received education, a detailed and personalised selfmanagement plan and follow up. Preliminary evaluation of this clinical service has shown: Extraordinary patient feedback 24% drop in the number of prescriptions for oral steroids 25% drop in the number of GP visits 100% drop in hospitalisations Patient Thank You – Click Image The Paediatric Research Team received this letter from a family that travelled from Wokingham for 4 visits, over 6 months to take part in the CLOTHES (Clothing for the relief of eczema symptoms) study. Spotlight on Research in the Emergency Department We look after patients with an acute illness or injury, carry out initial stabilisation investigations and provide treatment. Research is very well integrated into the Department as 98% of the 14 consultants are GCP trained. Denise Foord is the dedicated Research Nurse for the Emergency Department. We have regular teaching for doctors and nursing staff and this gives us an opportunity to raise awareness of the on-going studies we are currently recruiting to. We hope to build on the research team with the addition of a Clinical Trials Assistant (CTA) in the future. We are recruiting to 2 international randomised, double-blind, placebocontrolled trials: Clinical Randomisation of Antifibrinolytic in Significant Head injury Haemorrhage Alleviation with Tranexamic acid – Intestinal Tract (Diagnosis of Pulmonary Embolism in Pregnancy) a prospective diagnostic study Clindamycin for Cellulitis - a randomised controlled trial. Facilitating the Paramedic 2 trial (The Adrenaline trial) , Adrenaline vs. Placebo in pre-hospital cardiac arrest. Achieved our recruitment targets a year ahead of what has been agreed and are now over recruiting. We also recruit to non-ED studies including GARFIELD, FIRST and PRIME. We are the only Department in the hospital that recruit 24/7 even under these huge pressures. We are currently in discussion regarding 2 new studies: Rapid CTCA – The role of early CT Coronary Angiography in the evaluation, intervention and outcome of patients presenting to the Emergency Department with suspected or confirmed Acute Coronary Syndrome. BEST - Expediting the confirmation of acute myocardial infarction with point of care troponin testing to facilitate early intervention. Spotlight on Spotlight on Lisa Gadeke, Research Nurse I am a Specialist Research Nurse within Gastroenterology. I support the delivery of clinical trials in terms of collecting data and performing research related procedures on patients and I have been heavily involved in the development, implementation and delivery of new home-grown studies. This has lead to working with patients to capture their views and engage their support to our new ideas. Embedding research remains a clear focus for me across all areas of gastro medicine and nursing, and as a result, I spend a lot of time teaching and educating. Representing PHT at the Oesophageal Cancer campaign at Westminster last year with the Gastro Research Team and one of our patients who had received treatment as a result of one of our studies, was a high point. I think the patients’ voice is so powerful and it was quite humbling to be there with someone who not only had a very positive outcome to his treatment but had also undertaken his journey as research patient. I’m a keen runner and enjoy road biking whenever I get the opportunity. My idea of heaven is being miles away in the great outdoors running, walking or up high skiing down a mountain! Developing Public Patient Involvement is a clear goal for the Gastro Research Team and something I feel very passionately about. I hope to influence and develop Research ideas that have patient input and are driven by patients. We currently have some really exciting studies running, including the cytosponge study in the assessment of Barrett’s Oesophagus. We hope to launch a number of new home-grown, interventional studies including one called E-ring in the New Year. The study uses a novel disposable device that slips onto the end of conventional colonoscopes to enhance mucosal views and enhance tip control during colonoscopy. This will be another high volume recruitment study which we hope will be as successful as our recent E-cap study that recruited 534 patients in 12 months. The Clinical Outcomes Research Group (CORG) led by Dr Greta Westwood has been shortlisted for another HSJ award in the category, “Improving care with Technology”. Metapher – PHT have recruited the first and second patient nationally to this breast cancer study. He will feature in The Link and in a national journal. Non-malignant haematology – PHT were the top recruiter in Wessex with 45 patients recruited into 4 studies – ROSE, Garfield, First and UKITP.
