All graduate certificate students must complete this form to earn a graduate certificate. Submit the form to your Student Success Coach to route to the Registrar. If your
home campus is Online, please fax your form directly to 866-487-9582.
Certificate requirements are reviewed after you finish the last class required for the certificate. Your certificate will be mailed at the end of the semester in which you complete
the last required certificate course, after the final grade for your final certificate class has been received and all requirements have been satisfied.
