Third Sector Initiatives – Criteria For Applications

Intermediate Care helps avoid unnecessary hospital admissions, inappropriate admission to residential care and delayed discharge from hospital, requiring services which focus on prevention, rehabilitation, re-ablement and recovery

Welsh Government has developed an Intermediate Care Fund for 2014-15 only to encourage integrated working between local authorities, Health and other partners. The overall aim of the Fund is to support improvements in the management of short term unscheduled pressures and over a longer term, to support people to remain at home through improved levels of wellbeing.

As part of Flintshire’s successful bid to the Welsh Government, £100,000 has been allocated for third sector initiatives. Third Sector Organisations providing services in

Flintshire are invited to apply for funding to deliver against the priorities below. Funding is primarily targeted at supporting older people, particularly the frail elderly and we are looking to support approaches that help people to help themselves.

Priorities

A. promoting wellbeing and preventing/delaying the need for statutory services

B. providing appropriate support at the end of a service/programme such as reablement to help reduce social isolation e.g. supporting people to access community based activities, social networks, social media groups/networks.

C. developing/pilot new approaches to enable people to remain linked into, and part of their community e.g. developing and supporting dementia friendly localities, local area co-ordination, developing community resilience, and co-produced services.

Criteria

Please note that this is non-recurring funding, so any proposals must be designed with that in mind. We can not support proposals that create dependency and we are interested in initiatives that go beyond ‘9-5’ weekday’ boundaries. Organisations are advised to put forward proposals which can commence as soon as approval has been granted in June

2014 and demonstrate that money has been spent by March 31 st 2014.

Proposals will be assessed against the following criteria:

1. Is the aim of the proposal clear and does that aim support at least one of the listed priority areas above?

2. Are the activities of the project clearly described and relevant to the aim?

3. What rationale is given for why the project will be able to meet the identified need.

4. Are the differences (impacts/outcomes) clearly described and achievable by the p roject’s activities? Is the information identified to demonstrate ‘difference’ (a) appropriate and (b) collectable within the resources available to the project?

5. Is the amount of funding requested, appropriate for the delivery of the project’s activities / estimated number of beneficiaries / timescale?

6. Are services complimentary to existing service provision? Is there co-ordination between projects/organisations that brings added value?

7. Does the proposal describe an exit strategy and/or explanation of how the impact to existing clients/service users will be minimised at the end of the funding period?

A panel will meet on 2 nd June to assess applications. Further dialogue may be required with organisations in order to maximise the use of the funding available and the results will be shared as soon as possible by email.
