Human Factors Analysis Tools (HFAT) Course
Introduction
Human Factors Analysis Tools (HFAT) is a methodology for investigating and
analyzing human and organizational factors aspects of incidents and
accidents.
In 2008, the UK Energy Institute published an independent review of twentyeight such methods against a set of nine features deemed most useful to the
user. Only three methods, including HFAT, met all nine criteria.
HFAT was specifically designed to supplement a company’s existing
investigation methods, adding the ability to analyse and address human and
organizational factors.
The course provides delegates with the necessary skills and understanding to
use the HFAT toolkit.
Course Aims
The aims of the course are to:
 Provide a basic understanding of how human factors influence health and
safety behaviour and performance
 Enable experienced incident investigators to (a) better understand why
people involved in incidents behaved as they did, and (b) write more
effective behavioural recommendations which will positively influence the
behaviour of those immediately involved, and others, in the future.
Course Objectives
By the end of the course, delegates will be able to:
1. Describe key human factors topics, specifically safety culture, applied
behaviour analysis, ergonomics and human error
2. Identify which human factors topics influence people’s behaviours in HSE
incidents
3. Extract all the key behaviours, at all levels of the organisation, relevant to
each critical factor associated with an incident
4. Decide whether behaviours that are evidenced during an investigation
were intentional violations or unintentional errors
5. Using the ABC model, explain how violations are influenced by
antecedents and consequences.
6. Describe the basic types and underlying causes of errors
7. Construct recommendations which deliver antecedents and consequences
to influence the desired HSE behaviour
8. Employ a human error analysis (HEA) model to prevent or mitigate the
likelihood of future errors

Who Should Attend the Course?
Delegates should:
 Have a minimum of 2 years operational management and/or HSE
experience, which includes experience as a safety representative
 Be trained in incident investigation methodology to competent level, and
practised in its use
 Have a demonstrable desire to deepen their understanding and application
of human factors tools and techniques, and promote their use
 Satisfactory completion and submission of pre-course work (approx 3 hrs)
by the agreed deadline
 Be fluent in written and spoken English
Successful completion of the course will entitle delegates to use the HFAT
materials in their company only, and will not entitle them to copy the materials
for any purpose other than their personal use in their company, or to permit
people who have not completed the HFAT training course to use them.

expertise in ergonomics and psychology solutions for business. The company

Course Duration
The course is two days in duration.
Delegate Materials
Delegates will be provided with a delegate pack and memory stick which
include the teaching material, practical exercises, case studies and tools for
future use.
Intellectual Property
To protect the intellectual property rights of The Keil Centre Ltd, the following
clause should be noted.
All Intellectual Property in the materials and methods comprising the Consultant's
"Human Factors Analysis Tools", specifically training manuals and materials, printed
worksheets, electronic worksheet, case studies, and pre-course knowledge test, is
vested in the Consultant, and will not become the property of the Company at any
stage. On completion of the 2-day Human Factors Analysis Tools training course, the
trained delegates are permitted to use the materials, methods, electronic worksheet
on an unlimited basis in the Company, without charge or further payment of a licence
fee, but are not permitted to give the materials and methods to untrained employees
of the Company, or to any other person out with the Company.
