Spring seems to have brought with it all seasons in one - hope we have still some summer weather to come. This edition of the Spring newsletter provides an update on recent Change Plan submissions which were considered by the Ministerial Strategy Group in March. There is clearly a lot of momentum now with some excellent work being undertaken to meet the objectives of the Reshaping Care for Older People Programme. There is also work to do, including the important aspect of ensuring the commitment to carers is met. There is further detail in the newsletter on carer information strategy recent shared learning event. The Improvement Network continues to support shared learning across Scotland and we have received some helpful suggestions as to the focus for activities going forward. The newsletter provides an update on Change Plans related work on joint commissioning, community capacity, health and social care data, intermediate care and Talking Points. JIT is pleased to be working with Governance international on recent and forthcoming workshops and seminars on co-production and a number of partnerships are taking work forward locally. There will be further updates on this in subsequent newsletters and e-bulletins. Other updates include information on work being undertaken by QuEST focusing on innovation which I am sure has wider relevance to partnership working, the Scottish Community Care Benchmarking network and Telehealth and Telecare Network. Warm regards, Dr Margaret Whoriskey Director Joint Improvement Team 2 CHANGE FUND UPDATE The Reshaping Care for Older People (RCOP) programme and associated Change Fund is a key driver for health and social care integration and is supporting early implementation of the integration policy. The valuable learning to date and work underway will inform development and implementation of adult health and social care integration going forward. All 32 partnerships were asked to submit their Change Fund plans for 2012/13 by the middle of February. These were considered by the Joint Improvement Team (JIT) and by officials from Scottish Government, CoSLA, NHS Education for Scotland, Scottish Social Services Council, Scottish Council for Voluntary Organisations (SCVO), Coalition of Carer Providers in Scotland (CCPS) and Scottish Care. The resultant report was endorsed by the March meeting of the Ministerial Strategic Group. There are many positive themes emerging from this years plans, not least the fact that overall, in 2012/13, Partnerships are allocating 48% of the Change Fund to preventative, anticipatory and more responsive community based services aimed at supporting people at home and in their communities, with this estimated to shift to almost 60% in 2014/15. The consensus is that overall there is good progress being made although partnerships plans are still in the early stages of implementation, with many requiring time to further develop their relationships across NHS, Local Authority, Third and Independent sectors. However, this is building a solid foundation for effective partnership working and creating the conditions required to deliver on the ambitions of the programme. The third and independent sector partners emphasised the importance of reinforcing the requirement for robust Partnership Governance arrangements and transparent decision making with Third and Independent Sectors and other stakeholders. The Health and Community Care Delivery Group has established a sub group focusing on Improvement and Support and this will provide an opportunity to ensure coherence across range of improvement activities and effective targeting of existing resources. JIT is pleased to be working with LTCAS and partners (SCVO, CCPS, Scottish Federation of Housing Associations and Voluntary Action Scotland) as well as Scottish Care to support improvement and capacity building with third and independent sectors on Reshaping Care and implementation of the Change Plans. As well as considering market analysis they provided participants with an opportunity to consider the likely 3 impacts of Self Directed Support and Health & Social Care Integration for strategic commissioning. Watch the JIT website for the write-up and references. The next series which will take place on the 21st, 23rd and 24th May will be considering Strategic and Service Review – particularly focussing upon outcomes and user/carer led approaches. On the national front a new National Strategic Commissioning Steering Group has been established to support the MSG Delivery Group to identify and address the issues that were raised in the recent Audit Scotland report on Social Care Commissioning and to ensure that partnerships are best placed to implement their future integration plans. Jointly chaired by Alan Gunning, Chief Operating Officer of NHS Ayrshire & Arran and Peter McLeod, Director of Social Work at Renfrewshire Council and Vice President of ADSW, the group has a wide representation from all sectors and stakeholder interests. One of the Steering Groups first tasks will be to oversee the production of a Strategic Commissioning National Learning Framework. The Institute for Public Care at Oxford Brookes University has been commissioned to produce a framework which will provide a comprehensive picture of the learning and support needs of local partnerships and proposals for how they can be met. JIT Commissioning resources: http://www.jitscotland.org.uk/actionareas/commissioning/ UPDATE ON THE INTEGRATION AGENDA Planned legislation to integrate health and social care will change the way care is planned and provided to better deliver support and care to people. Scottish Government will conduct a broad, formal consultation on the proposals in May 2012 and will host a series of events across Scotland to seek stakeholders’ views on what is proposed. The Change Fund for older people’s services is already focusing on preventing delays and shifting care for older people upstream to community based services. The total fund of £300m is split as: £70m/80m/80m/70m over the 4 years from 201112 to 2014-15. We continue to work closely with partnerships to ensure this money is put to best use in support of positive outcomes for patients, clients, carers and their families across Scotland. The day was focused on the 4 significant contribution to be made through the home environment and the housing sector in the Reshaping Care for Older People Programme. For more information on the Improvement Network, contact Margot.White@btinternet.com. 5 CO-PRODUCTION AND COMMUNITY CAPACITY UPDATE NATIONAL CONFERENCE The Joint Improvement Team held a highly successful national conference focusing on Community Capacity and Co-production in Dunfermline on 26 January 2012. The event which was attended by over 200 delegates from across the statutory, Third and non-statutory sectors was opened by Councillor Douglas Yates, COSLA Spokesman for Health and Wellbeing and received a message of support from the Deputy First Minister and Cabinet Secretary for Health, Wellbeing and Cities Strategy. The conference also heard an affirmative message from Derek Feeley, Director General for Health and Social Care and NHS Scotland Chief Executive on the relevance of this agenda for public services. The event showcased innovative examples of local partnership working and successful co-production initiatives such as the Family Nurse Partnership, Scottish Violence Reduction Unit and NHS Fife’s SHINE Project. Delegates also learned about efforts to better evidence the impact of these approaches through the use of Contribution Analysis and heard about new training opportunities in the use of Governance International’s Co-production Toolkit Full details of the presentations and other resources from the day can now be downloaded from the Co-production area of our website at http://www.jitscotland.org.uk/news-and-events/ GOVERNANCE INTERNATIONAL CO-PRODUCTION TOOL KIT The success of our January training events in the use of the Governance International Co-production Toolkit has generated significant interest among partnerships to the extent that JIT is delighted to announce the availability of a further three 1 day training opportunities over the period 16 – 20 April 2012. On the basis of information that Partnerships have kindly provided to us we are currently identifying examples of good practice that we can use as case studies to build a fuller picture of achievements and success to date. To make sure we create accurate and informative case studies the National Leads together with JIT Action Group members intend to visit all 32 Partnerships during 2012/2013 to see examples of progress in action, capture the associated learning and enable both learning and progress to be shared across all Partnerships. Following a sort consultation period, the draft is now in the final editing stages, with a view to publishing in May. Comments on the draft have been received from a wide range of stakeholders, from across health and social care, professional bodies, and the third and independent sectors, and are now being used to shape the final version. Although the consultation has now closed the draft can still be viewed at http://www.scotland.gov.uk/Topics/Health/care/17420/IntermediateCare/ICFramewor k. We are also still keen to receive any examples of good practice around Intermediate Care, particularly those involving the third or independent sector. JIT intermediate care resources: http://www.jitscotland.org.uk/actionareas/intermediate-care/ 7 TALKING POINTS There is a lot of activity ongoing around Talking Points personal outcomes approach. Talking Points has recently been reviewed by Alison Petch, Director of IRISS, who found that the approach demonstrates major strengths, including successful user engagement and improved professional understanding of outcomes. The report will be available in April. To support ongoing implementation by partnerships and providers, the JIT is currently concluding work on the ‘Talking Points: Practical Guide’ which will be available in May. A data retreat to progress understanding of measuring and using outcomes data was held at the University of Edinburgh in March 2012, supported by the Community Care Benchmarking Network and IRISS and attended by the JIT, Care Inspectorate and many partnerships and providers. The report will be available on the JIT website in April. There are workshops for providers in May and June, including an introduction for organisations new to the approach, and a further workshop on outcomes focused conversations. The Community Care Outcomes Framework has recently undergone review, with a key recommendation being to strengthen the focus on personal outcomes. Local organisations and partnerships have recently made significant contributions to the continuously developing outcomes approach, and these will shortly be reported in a bulletin which will be posted on the JIT website. JIT Talking Points resources: http://www.jitscotland.org.uk/action-areas/talkingpoints-user-and-carer-involvement/ QUEST OUTPATIENT INNOVATION SESSION In January the Quality and Efficiency Support Team in partnership with the Duncan of Jordanstone college of Art & Design held a Future Focus Session on looking at Transforming New Demand for Outpatient Services across NHS Scotland. A group of approx 30 people including GPs, consultants, service and operational managers, planners, NHS 24, Scottish Telehealth Centre and Service Users from Forth Valley and Tayside were invited to participate. Elaine Mead, Chief Executive, NHS Highland opened the session with the aim to generate concepts to reduce new demand for outpatient services that will be taken on to prototype, test and be implemented Scotland wide over the next 18 months. The day utilized the RIP+MIX a design method, which was developed by T-Labs, Deutsche Telekom and a research team from the University of Dundee. It is a fast, effective and engaging design tool to help encourage creativity and innovation in both designers and non-designers. The simple analysis tools codify parts of the design process which are often left to intuition. 8 In groups, the delegates looked at the parts of the outpatient service that gives most “pain” the areas that could be improved and mixed it with something that gave us “pleasure”. For example, there was a painful experience (long waiting times) mixed with a pleasurable experience (tracking an online order from Amazon) generating the idea that patients could be tracked similarly and came up with a proposal to investigate if this is a feasible idea. This was a new, exciting and innovative technique to generate ideas and was very ambitious on the day. We came away with several proposals and are looking at bringing the group together again in May to do some more sharing of what’s already happening. Some of the ideas are already in action in some Boards and we need to action plan to spread and develop our vision for the future. QuEST website: http://www.scotland.gov.uk/Topics/Health/NHS-Scotland/DeliveryImprovement SCOTTISH COMMUNITY CARE BENCHMARKING NETWORK UPDATE The Scottish Community Care Benchmarking Network (SCCBN) is a network of 31 Health and Care Partnerships in Scotland. It has a structure, governance a partnership agreement. It is funded by Partnership’s subscription and project funding from the JIT. It is supported by a Co-ordinator and Research Assistant who provide a Benchmarking Service which can: Collect, analyse and present specific datasets Facilitate and co-ordinate specific benchmarking projects Carry out research on specific topics Provide regular benchmarking reports Provide benchmarking training A web-based data management system and benchmarking query tool is available to Partnerships to carry out their own data benchmarking. In January, SCCBN members attended Benchmarking Training – delivered by Quality Scotland. The training provides an understanding of the purpose and principles of benchmarking, increases an awareness of the skills needed for effective benchmarking and provides methodology to follow a systematic approach. At its Quarterly Meeting in February, there were presentations on; 9 The SOLACE Benchmarking Project, Developing an Integrated Health and Social Care Outcomes Framework, Talking Points Review and Action Plan, The capture, analysis and use of information on service user and carer outcomes in health and social care settings. A SWOT analysis of the Network was also undertaken to agree on how the Benchmarking Network can best support Partnerships in the coming financial year. The Faster Access Benchmarking Project Group undertook a site visit to West Lothian on 26th March, on a peer review basis, to understand ‘what works’ in relation to delayed discharge processes. Visits to East Ayrshire and Moray are being arranged for late April. A report on these visits is planned for circulation in early May with discussion at the Quarterly Meeting, later that month. This has been derived from publicly available national data sources and its primary purpose is to inform and advance the thinking of Partnerships regarding their local Change Plans. The spreadsheet is fully interactive with tables and charts which allow for comparisons with up to six different areas of your choice. The aim was to provide an opportunity for NHS Boards to showcase their own good practice and learn from others with a view to adopting and/or adapting to suit their local needs in identifying and supporting carers and young carers. This attracted delegates from across Scotland. Robert Stevenson Senior Project Manager, NHS Forth Valley gave a strategic overview of workforce development in FV. The Minister for Public Health, Michael Matheson MSP delivered a speech entitled “Supporting Carers and Young Carers” in which he reaffirmed the Government’s commitment to identifying and supporting carers and young carers. The Minister highlighted that at least 20% of the Change Fund is to support carers of older people. The day was interactive and information packed with presentations, workshops and a marketplace of 20 stands, all demonstrating outcome focused good practice in identifying and supporting carers and young carers, made possible by CIS funding. The event programme will include focussed World Café style discussions around key topics which might include workforce development and education, standards, procurement, governance, etc. The purpose of the focussed discussion is to gather views on common issues relating to telehealth and telecare deployment which can be used to inform the national strategic agenda and work programmes.
