Appointments - Same Day or Book in Advance
We have divided our appointments into two categories: 'Same day' and 'Pre-bookable'.
Same day appointments are released at 8.00am each morning and enable you to telephone and
see a doctor that day. We will endeavour to make the appointment with the doctor of your choice
although that is not always possible.
If you wish to see a particular doctor or to make your appointment for a specific date then one of
the pre-bookable appointments may enable you to plan your visit accordingly. However, with the
doctors' other NHS commitments, this may mean booking a little way in advance. If your own
doctor is unavailable please do not put your health at risk by unduly delaying your appointment.

Appointment Times
We offer a range of appointment times every day starting at 08:00 with both pre-bookable and
book on the day appointments.
Express Clinic
We also have an 'express clinic' every morning for all minor ailments, and these appointments
can be booked on the day (from 08:00) and start at 08:30.
Pre Booked Extended Clinics
We also offer some pre-bookable appointments outside of our normal opening times, specifically
to try and assist those who have difficulty in attending during normal opening times. These are
on varying days each week and the surgeries start at 07.00 and another at 18.30. Also on a
Saturday starting at 08.00. Please see the notice in reception or telephone for details.
These are for pre booked appointments only and there are no other services available.
Telephone Advice
We provide a telephone consultation service. Please ask the receptionist for details.
Home Visits
If possible please try to telephone reception before 10am if you require a home visit.
Home visits are only available for patients who are housebound because of illness or disability
and are always at the discretion of the doctor.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by
telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a
hospital attendance.
A rash or temperature does not prevent patients coming to the surgery and will not endanger
others. Simpl;y tell the receptionist on arrival at the surgery.
Please remember that several patients can be seen in the practice in the time that it takes to make
one home visit. There are also better facilities for examining and treating patients at the Health
Centre.
The Surgery
The building is purpose built with consultation rooms located on two floors and two separate
waiting rooms.
On arrival please book in at reception on the ground floor using our automated arrivals system.
This will confirm which clinician you will be seeing and inform you of the appropriate waiting
room to use, the Ground Floor or First Floor.
When the doctor or nurse calls you to the consulting room your name and the number of the
consulting room will be displayed on the electronic patient information board in each waiting
room.
Chaperones
Some examinations that patients have to undergo can cause embarrassment and anxiety, and
patients may prefer to have someone else present.
Following guidance from all the professional bodies, we will be offering and encouraging the use
of chaperones in appropriate situations. You may decline the offer of a chaperone although in
some circumstances the doctor may feel that a third person should be present and will discuss
this with you if the need arises.
Repeat Prescriptions
Requests for repeat prescriptions should be made at least two working days (48 hours) in
advance, when the request is made at the surgery and three working days (72 hours) when the
request is made at a local Crowborough pharmacy. Please remember to allow sufficient time at
the weekends and bank holidays.
Requests may be made in person, in writing (with a stamped, addressed envelope) and by fax.
Telephone requests are not accepted.
The doctors recommend the use of the prescription collection service for repeat prescriptions.
This facility is available from the local pharmacies in Crowborough. Please speak with your
usual pharmacist if you wish to use this service.
It is most important that your medicines are reviewed at least once a year with the doctor. You
may wish to use your birthday as a reminder to see the doctor that month.

Health Promotion
Preventative medicine including heart disease prevention is undertaken by all the doctors with
the assistance of the practice nurses and health care assistants. There are a number of specific
health promotion clinics available.
Diabetes
Run by Dr Slatter and Sister Caroline Thoms.
Asthma
Run by Sister Caroline Thoms.
Coronary Heart Disease
Run by Alison Cantwell.
Cervical Screening
It is important for all women between the ages of 25 and 64 years to have regular cervical smears
- appointment recalls will be sent out automatically at the appropriate intervals. This procedure
can be performed by the practice nurses.
Pregnancy
If you are pregnant either contact the surgery to see or speak to a doctor or telephone the
Crowborough Birthing Centre on (01892) 664643.
Contraceptive Services
Each doctor is willing to provide full contraceptive services including post-coital contraception,
and any doctor or nurse can deal with this during a routine surgery consultation. Anyone
requiring an IUCD (coil) will be referred to Dr Morris who specializes in this field.
Immunisations
Appointments for routine childhood immunisations are sent out automatically by the Health
Authority. Other immunisations, such as those for influenza, are also available, and our practice
nurses will be happy to advise regarding all immunisations. It is extremely important that all
children are immunised appropriately.
Flu Vaccination And Pneumonia Vaccine
An influenza vaccination is particularly recommended for patients with heart, lung or kidney
disease, diabetes, residents of nursing and rest homes and retired people. Please contact the
reception staff in October for details of the clinic dates and to make an appointment.
Mammography - Breast X-Ray
We advise all women to check their breasts regularly and report any abnormalities to a doctor.
All women between the ages of 50 and 70 years will be invited to attend the breast screening unit
for a mammogram. You will be notified of your appointment when the unit is in this area. We do
urge you to take advantage of this offer. Screening does save lives.
Women outside this age range can have a mammogram where appropriate and if you are
concerned please discuss this matter with a doctor.

Minor surgical procedures are carried out at Crowborough War Memorial Hospital.

Influenza And Pneumococcal Vaccine
In accordance with Department of Health guidelines, we recommend an influenza plus
pneumonia vaccination for patients over 65 and those patients under 65 with diabetes, chronic
heart, lung or kidney disease and residents of nursing and rest homes.
The influenza vaccination is generally available in October please contact the practice for details
of clinic times etc.
The pneumococcal vaccination is available throughout the year.
Non NHS Services
Medical Examinations
Medical examinations for special purposes, such as fitness to travel, pre-employment, insurance,
driving medicals, fitness to undertake specialised sports and hobbies etc are not NHS services. A
fee will be payable. Please contact the surgery to make an appointment.
Letters and Forms
Letters to employers, schools and universities (not statutory medical certificates), insurance claim forms,
passport applications etc are not NHS services.
Travel Vaccinations
Some travel vaccinations although recommended are not available as an NHS service - the nurse giving
travel advice will give you of details of any costs.

Full details of the services we provide and other helpful information is included in our Practice
booklet and on this web site, please take the time to familiarise yourself with the way we have
organised the practice. In particular the sections on Appointments, Foreign Travel and Repeat
Prescriptions, this will save you time and help you to get the most from our services.
Registration Process
Following patient feedback we have amended the registration process so that in most cases
patients do not have to attend for a new patient check. However this does mean that all the
information requested must be supplied on the forms which can all be downloaded from the preregistration tab.
In order to register please;




Complete the Health Authority form
Complete the Ethnic Monitoring form
Complete the Health Questionaire form
Hand the forms to the receptionist together with
o Original documentation showing Photo ID (Passport, driving licence, ID card)
o Evidence of address (council tax bill, paid utility bill etc)
o a urine sample labelled with your name and date of birth
o a BP reading from the machine in the waiting room if you are over 18. Please ask
if you require assistance
o your medical card – if available.
Please note that we will not be able to register you as a patient unless all the information
requested on the forms is provided.
In our experience investing a small amount of time now on your health will ensure that we are
better able to meet your future needs.

When We Are Closed
Out of Hours
Between 6.30pm and 8.00am weekdays, all weekend and on bank holidays you can access urgent
health care via NHS 111 and South East Health. Telephone 111 - this is a free call. Your call will
be answered by trained operators who will arrange the most appropriate action for you. This may
be a phone call from a doctor, a trip to a primary care centre or a visit from a mobile doctor.
Calls are recorded.
Please ensure that your medical need is urgent and cannot wait until the surgery is next open.
Routine requests for medical attention just delay the response to those whose need is more
pressing.
South East Health have been commissioned to provide the out-of-hours cover in this locality.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery
re-opens.

Minor Injury
There is a Minor Injury Unit (MIU) at both Crowborough and Uckfield Hospitals available daily
from 8.00 am to 8 pm. This maybe a more appropriate service to use than A&E and it could have
shorter waiting times
Call 999 in an emergency. Chest pains and / or shortness of breath constitute an
emergency.
Foreign Travel

Our practice nurses will be pleased to give advice to those patients going abroad. Please allow
adequate time in order for the vaccine(s) to be effective. Certain vaccines are chargeable - please
ask the nurse for details.
Before You Go
Well before travelling abroad, check with your travel agent or the tourist office/embassy of the
country you intend to visit on any special precautions you may need to take. At least two months
before departing discuss any vaccination requirements with the practice nurse. Pack a small first
aid kit containing adhesive dressings, insect repellent, antiseptic cream and water purification
tablets etc; ask at the pharmacy.
When Abroad
Check on the quality of the drinking water. If in doubt, either drink only bottled water or use
water purification tablets. Avoid ice in drinks as this may well have been made from suspect
water. Raw vegetables, salads and fresh fruit should be carefully washed in clean water. Again, if
in doubt, stick to freshly cooked food.
Beware of the sun! Use a high factor sunscreen especially in the first few days of exposure.
Children in particular should be closely monitored.
In hot climates, drink plenty of non-alcoholic drinks. If you are not passing water regularly you
are not drinking enough.
On Your Return
If you fall ill tell your doctor that you have travelled abroad and what countries you have just
visited. If you have had treatment abroad tell your doctor. When donating blood tell the
transfusion staff which countries you have visited.

the services provided by the NHS.

Test Results
Please call between 2:00pm and 4.30pm to enquire about your test results as our reception staff
will have more time to deal with your request at this time.
The practice has a strict policy regarding confidentiality and data protection and we will only
release test results to the person to whom they relate unless that person has given prior
permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the
practice. It is your responsibility to check your results and to make an appointment to discuss
them with your doctor if your are advised to do so.

Practice Policies
Confidentiality
You provide us with personal information so that we may provide you with the appropriate
treatment and care. Information is recorded on computer and we are registered under the Data
Protection Act.
Patient confidentiality will be maintained at all times although, to ensure the most effective
functioning of a multi-disciplinary team, it is sometimes necessary to share medical information
between team members, all of whom are bound by a code of conduct.
Patients' Rights And Responsibilities
You will be treated with courtesy by all team members, who will be skilled in dealing with your
enquiries and problems.
Please treat all our staff with courtesy and respect.
All team members will respect your confidentiality. Your records, both written and computerised
will be kept secure and confidential. If you wish to see your medical records please ask any
member of staff. A fee may be payable.
We will honour your religious or cultural beliefs.
Please let us know if you cannot attend an appointment, we can then free this appointment for
another patient.
Complaints Procedure
Frank Powell, our practice manager is available by appointment to discuss any complaint you
may have against the surgery. You may also make a written complaint, addressed to Mr Powell.
This should be submitted within a few weeks of the incident. The practice will endeavour to
investigate any complaint up to 12 months after the alleged incident.
Strict rules of confidentiality will be observed. A complaint made on behalf of someone else
must be accompanied by that person’s signed agreement to the complaint whenever possible.
An acknowledgement will be sent within three working days and may include an offer of an
interview to clarify the problem. If necessary there will be an investigation, following which an
invitation may be issued to both concerned parties to meet. Alternatively, a written response will
be sent within 25 working days, stating any conclusions and recommendation. The complainant
will be notified should there be a delay in responding.

PALS
PALS (Patient Advice and Liaison Service)
(www.pals.nhs.uk/) The NHS employs over a million staff in thousands of locations. It is a large
and complex organisation providing a broad range of services. It is not surprising that sometimes
you or a loved one may feel bewildered or concerned when using the NHS. And this can be at
times when you are feeling at your most vulnerable and anxious.
So, what should you do if you want on the spot help when using the health service? The NHS
expects all members of staff to listen and respond to you to the best of their ability. But
sometimes, you may wish to talk to someone employed especially to help you. The Patient
Advice and Liaison Service, known as PALS, has been introduced to ensure that the NHS listens
to patients, their relatives, carers and friends, and answers their questions and resolves their
concerns as quickly as possible.
PALS also helps the NHS to improve services by listening to what matters to patients and their
loved ones and making changes, when appropriate.
What does PALS do?
In particular, PALS will:







Provide you with information about the NHS and help you with any other health-related
enquiry
Help resolve concerns or problems when you are using the NHS
Provide information about the NHS complaints procedure and how to get independent
help if you decide you may want to make a complaint
Provide you with information and help introduce you to agencies and support groups
outside the NHS
Inform you about how you can get more involved in your own healthcare and the NHS
locally
Improve the NHS by listening to your concerns, suggestions and experiences and
ensuring that people who design and manage services are aware of the issues you raise
Provide an early warning system for NHS Trusts and monitoring bodies by identifying
problems or gaps in services and reporting them.

Tips on evaluating the reliability of online medical information
1. Use sites from reputable organisations you have heard of.
2. Double check the information by looking for other sources.
3. Check there are contact details for any organisation or people responsible for the
information not just an email address.
4. Look at the advertising - does it seem to unduly influence the site content?
5. All articles should quote references that you can look up.
6. Watch out for claims about the superiority of any particular treatment over another
7. Is this information supplied with a date to make sure it has not been superseded by other
research.
8. Check the sites confidentiality policy.
9. Beware of sites claiming to be the definitive source.
10. Beware of sites that criticise others.

Community Nurses
The team of community nurses care for patients who need nursing at home and are
based at Grove House, Crowborough. Messages can be left at the surgery reception
or by calling the surgery on (01892) 652233 before 11.00am.
Health Visitors
There are health visitors working closely with the practice. They are nurses with
extra training in the care and development of children under five years old. They
have special responsibilities to mothers and children and are based at Grove House,
Crowborough.
Counsellors
We have two trained counsellors attached to the practice who may be seen by prior
referral from your GP.
