The portfolio should be used in the in the community setting with your district nurse mentors in addition to the practice portfolio used with your medical mentor for your non medical prescribing module. Non- Medical Prescribing V300 In order to meet NMC guidelines for the District Nurse course, you are required to undertake a total of 22 practice days per trimester, the non- medical prescribing module runs over two trimesters. The practical component of the non medical prescribing course that you are required to fulfil by the NMC is a total of 78 hours over the two trimesters. This equates to approximately 12 days which is included in your 22 days per trimester. This means over the next two trimesters from January 2015 until August 2015 you need to complete 44 days in practice, 78 hours of this, must enable you to meet the NMC requirements for your non- medical prescribing course, the remaining 32 days will be spent under the direct or indirect supervision of your district nurse mentor. Community Nurse Prescribing V100 In order to meet NMC guidelines for the District Nurse course you are required to undertake a total of 22 days practice per trimester. The Community Nurse Prescribing module runs over 1 trimester, the practice component of the Community Nurse Prescribing course that you are required to fulfil by the NMC is 25 Hours which equates to approximately 4 days which is included in your 22 days practice per trimester. The remaining 18 days will be spent under the direct or indirect supervision of your district nurse mentor. You will use this additional practice portfolio to demonstrate your growing competence in your Specialist Pathway. Practice will be evidenced in the portfolio by working with your mentor, and identifying relevant practice issues and suggesting or implementing changes within your supervision records. You are also expected to identify your days in practice in the portfolio. Please use this time as an opportunity to broaden your horizons, spend time with other practitioners and organisations who deliver health and social care Mentors/Practice Teachers will assign a pass/ fail mark to the portfolio at the end of the practice experience. You will be facilitated during the module by a member of the community nursing Team who will act as the link tutor between Anglia Ruskin University and practice. Tutorials will be available during the period in practice, which will facilitate this linking between theoretical concepts and practice. In relation to the practice period of the module you have been provided with this practice document which you will need to utilise with your mentor throughout the module to monitor your progress in practice on a fortnightly basis and to explore your growing competency. 3 At the formative Practice Assessment and at the Summative Practice Assessment the mentor/practice teacher, will review your progress. At any point during the prescribing course, if the practice teacher/ mentor has concerns in relation to your progress a Cause for Concern meeting can be initiated between you, the mentor and a member of the community nursing team. If you have any concerns that cannot be dealt with between yourself and your mentor you can ask for a Cause for Concern meeting to be initiated. During the ongoing assessment and supervision, the mentor will discuss with you the issues that they have identified, thus intellectual synthesis will take place using learning experiences in conjunction with the learning outcomes of the module. A member of the community nursing team will also arrange a Link Visit in the practice area once a trimester to address any issues that you or your mentor have. Support in the Practice Placement Support for students will be provided by the community nursing team and professionals working within the health care organisations and other organisations. Experience and support in practice will be located within the widest possible framework and include multidisciplinary teaching, supervision and mentorship. Contact will be made with innovators in practice across all involved organisations and incorporate national and international perspectives. Practice teachers/ mentors are closely involved in facilitating your practice experience, to help you meet the learning outcomes and competencies of the module. You will therefore need to give your Practice Teachers/ mentors access to VLE in order that they can use it when helping you to achieve the learning outcomes and competencies of the module. You can give your Practice Teacher/mentor access to the VLE for example during your fortnightly supervision sessions. Practice Teacher’s/ Mentor’s Role Student's Responsibilities You are expected to attend the practice placement each day for the normal working day period agreed by your mentor/ practice teacher. In the event of absence, the course Leader (Kellie Norris) should be informed. Similarly you must inform your Mentor/Practice Teacher when you are unable to attend practice. It is your responsibility to: 1. Negotiate and arrange your learning experiences with your Mentor/ Practice Teacher. 2. Arrange individual discussions/tutorials with your Mentor/ Practice Teacher in order to diagnose your learning needs and design action plans to develop competencies and meet targets. 3. At least once a fortnight throughout this experience a meeting must be arranged between yourself and your mentor/ Practice Teacher so that your progress can be assessed. N.B. It is a requirement of the course that your mentor/Practice Teacher sees your portfolio on a fortnightly basis. If this is not submitted to the Mentor/Practice Teacher they will 4 be required to inform the course leader and an action plan will be developed to ensure that this occurs. If after this meeting your portfolio continues not to be ready for the supervisory meetings a cause for concern meeting will be held, between yourself, your Mentor/Practice Teacher and Course Leader. 5 LEARNING AGREEMENT In order to make explicit the requirements of the student and the Mentor/ Practice Teacher in practice learning and assessment, it is essential to develop a learning agreement or learning contract. This is a tool to clarify and acknowledge issues likely to affect the learning experience and to provide a framework for dealing with changes and developments. Although there are areas that are usually a part of such a tool there should be room to work jointly with students to bring in their ideas and concerns. As adult learners students have specific skills and needs which must be incorporated into the planned learning experience. As experienced professionals working with clients, the Mentor/Practice Teachers have a variety of responsibilities to clients and to the employing organisation as well as the students. The learning contract should build upon placement preparation and should address: Basic contact details and agreement about attendance, time worked each day, absence and cover; Arrangements for supervision and progress review; Recognition of the power differentials between mentors/practice teachers and students; Commitment to anti-discriminatory and anti-oppressive practice; Recognition of the student learning style, previous experience and any difficulties or requirements expressed; Assessment of student’s learning needs based on previous experience and skills; Managing the transition from practising under supervision to independent practice. Recognition that if problems occur specific actions must be taken in partnership with the University; Client contact and confidentiality issues. 6 Assessment of Practice Mentors/Practice Teachers are responsible for assessing your competency to practice as a Specialist Community Practitioner District Nurse and will therefore assess practice experience and award a pass/fail at the end of the experience. The Final Assessment involves yourself and your Mentor/Practice Teacher only. The course leader is not normally involved in this process unless there is some concern about your practice. For the V300 Non Medical Prescribing this portfolio should be handed into the I centre by Monday 20th July before 14.00hrs. For the V100 Community Nurse Prescribing Course this portfolio should be handed into the I centre by Thursday 2nd April before 14.00hrs. OUTLINE OF TEACHING LEARNING AND ASSESSMENT TOOL Instructions for Use At least once a fortnight throughout the time you are undertaking the NMP or Community Prescribing Course a meeting should be arranged between yourself and your Mentor/Practice Teacher so that your progress can be assessed. A record of these fortnightly supervision meeting will be kept in this portfolio along with 3 User Testimonials. You will refer to your Mentor/ Practice Teacher as it is recognised that the patients/client groups are still the responsibility of the Mentor/Practice Teacher during this period. You will report to your Mentor/ Practice Teacher on a fortnightly basis for progress review and action planning. All supervision records must be up to date for these meetings. Assessment of relevant skills by your Mentor/Practice Teacher will take place on a fortnightly basis and relevant documentation completed. Final Assessment in Practice with your Mentor/ Practice Teacher will take place in the week that the portfolio is due to be completed. In order to ensure that clients have the opportunity to reflect on the work undertaken by you and provide feedback, clients will be encouraged to complete a testimonial on your work with them. Clients should be given the testimonials by the Mentor/Practice Teacher either in the Home or Clinic setting for completion. They can also contact clients by phone to receive feedback and complete the documentation. These will form part of your practice portfolio and part of the assessment in practice process. You will need to have at least three testimonials by the end of the module. The submitted portfolio must include: A learning agreement Practice Experience Log of Days Worked Mentor/Practice Teacher and Student fortnightly Supervision records Final assessment documentation User Testimonials (at least 3) GUIDELINES FOR ASSESSMENT PROCESS Diagnostic Assessment/Learning Agreement and Action Plan Process 7 Within the first week of practice your Mentor/ Practice Teacher and yourself will meet together to identify learning needs and develop an appropriate Action Plan. Your learning and progress during the practice experience will be reviewed on a fortnightly` basis with the Mentor/Practice Teacher during a clinical supervisory session. You and your Mentor/Practice Teacher will complete the appropriate documentation. In the event of your Mentor/ Practice Teacher or you being concerned as to the progress made, it is the Mentor/Practice Teachers responsibility to immediately contact the specialist lecturer within the University to arrange a date whereby the Mentor/Practice Teacher/Student and Specialist Lecturer/course Leader can meet to address the difficulties and determine an action plan (see – Cause for Concern Meeting documentation). Supervision records completed since you last submitted a portfolio, along with a log of days worked in that period, in consolidated practice and user testimonials should be attached to the portfolio. The portfolio should be read and comment sheets completed by yourself and your Mentor/ Practice Teacher prior to the meeting to sign off your District Nursing practice days. Assessment Boards and Results Ratified results will be available via E-Vision – address http://e-vision.anglia.ac.uk after the Awards Board.
