Deer Valley Lodging is pleased to offer guests attending The Clinical Aphasiology Conference, 5/25/2004 – 5/30/2004, the
following lodging options at The Lodges at Deer Valley.
The Lodges at Deer Valley is a deluxe condominium hotel and conference center that embodies the western spirit with a
rustic, lodge style ambience. A twenty-four hour front desk, concierge and bell services, underground parking, common area,
year round outdoor swimming pool and hot tub are just a sampling of the amenities that you will experience during your stay
at The Lodges at Deer Valley. The Lodges at Deer Valley is also on the complimentary citywide shuttle system offering
dining and shopping opportunities on Park City’s historic Main Street, 3/4 mile from your accommodations. Please note that
The Lodges at Deer Valley does not offer an on-site restaurant or lounge and is a non-smoking property.
A limited number of the following room type(s) are being held for this conference and will be taken on a first come/first
serve basis, subject to availability at the time of booking. Reservations must be made using this form. This room block will
be released on 4/25/04. Reservations will then be taken on a space available basis. Requests for specific bed configurations
will be noted.

BELOW RATES DO NOT INCLUDE 10.35% TAX (subject to change), 2.85% RESORT LODGING FEE OR $5 DAILY

ROOM/SUITE OPTIONS
HOTEL ROOMS – Each room features king, queen, double queen or double double bed configurations, full bath, cable
television, direct dial telephone, refrigerator, microwave, and wet bar. Approximately 400 square feet. Note: Very few
single hotel rooms available.
Nightly Rate: $105.00
ONE BEDROOM SUITES-Each suite offers state of the art, fully equipped kitchen, living area with fireplace, cable
television, direct dial telephone, washer/dryer, two baths, bedding varies. Approximately 700 square feet. Note: Well
worth the cost difference in comparison to the hotel room.
Nightly Rate: $125.00
TWO BEDROOM SUITES- Features the same configuration as the one bedroom, with an additional sleeping room and
bath. Approximately 1200 square feet.
Nightly Rate: $265.00
THREE BEDROOM SUITES- Features the same configuration as the two bedroom, with an additional sleeping room and
bath. Approximately 2,000 square feet.
Nightly Rate: $285.00
*PLEASE NOTE that all suites are designed with a common living room, dining area, kitchen and extra bathroom. Privacy
should not be a concern with the suite options; each bedroom in the suite is comparable to a single hotel room (with its own
bath and TV) and can be locked off from the rest of the suite. The suite options offer all of the potential seclusion of a
single hotel room plus the benefits of having access to a full kitchen, dining and living rooms.

Deposit/Cancellation policies:
An initial deposit of one night’s room and tax is due at the time of reservation by the individual guest.
Each guest will be charged a nightly $5 housekeeping/bellman gratuity and a 2.85% resort lodging fee.
Final payment will be charged upon departure.
Individual cancellations made more that 30 days prior to arrival will forfeit a $50 processing fee.
Cancellations made between 30-15 days prior to arrival will forfeit one night deposit.
Cancellations made within 14 days of arrival will forfeit total lodging cost.
NO SHOWS, EARLY DEPARTURES, LATE ARRIVALS, OR DOWNGRADES ARE SUBJECT TO FULL PAYMENT
OF CONFIRMED RESERVATION UPON DEPARTURE.

GROUND TRANSPORTATION – The Salt Lake International Airport is located 40 minutes from Deer Valley. Park City
Transportation offers 10 passenger door to door shared van service - $52 per person/round trip. Gratuity is not included in
this price. Payment for this service will be added to your final payment.
Cancellation Policy: Cancellations made within 24 hours of arrival will forfeit full prepayment of transfer cost.
