Advocate Health Care offers the opportunity to save money through two valuable discount programs.
Cellular Phone Discounts
As an Advocate associate, you are eligible for a significant discount if you have cellular phone service with AT&T,
Sprint, Verizon or T-Mobile. When you sign up through one of these cellular vendors for the Advocate associate
discount program, you can get a discount up to 25% off the cost of your monthly cellular phone service (depending on
your cell phone company).

These discounts are offered directly through the vendors—AT&T, Sprint, Verizon and T-Mobile. To take advantage of one of these discount
programs, you must contract directly with the appropriate vendor. Depending on where you live, an authorized dealer for AT&T, Sprint,
Verizon or T-Mobile may be offering other discounts on services; to ensure that you get the best deal, check out the authorized dealers in
your area before signing up for this program. These discounts are not part of the PerkSpot discount program Advocate offers.
To start receiving a discount on your cellular phone services, here’s all you need to do:

is nearest your home. A copy of the AT&T Customer Application for Discount form is available on www.advocatebenefits.com (go to Benefits Information > Discounts).
When you enroll, you will need to provide proof of Advocate employment, such as your associate ID badge, email
address, work phone number or check stub.
If you have cellular lines on your account for family members, these accounts may apply to the cost of their service,
as well—as long as you are the primary account holder.
Perkspot Discounts
And that’s just a sampling. For more information—or to
take advantage of discounts and rebates and others like
them that are available through PerkSpot—you need to
register on PerkSpot’s web site for Advocate associates:
www.perkspot.com. Once you do, you can:
•• Check out the deals each vendor offers. Simply click
on a vendor’s icon on the PerkSpot web site. Be sure
to make note of the instructions provided; you will
need these to “check out.” In some cases, you may
need to print a coupon. In others you only need to
make note of an offer code or present your
Advocate ID/Access card when you go shopping.
Note: PerkSpot discounts with some vendors will be
available only on merchandise and services purchased
through selected online stores.
•• At a vendor’s store—whether online or in person—pick
out the merchandise products or services you want.
Then follow the instructions provided on, PerkSpot to
check out. Note: You’ll be able to access the online
store of most vendors through links from the PerkSpot
web site.
•• Whatever the deal—whether a discount or rebate—
you’ll be credited with the savings when you check out.
