SUBJECT: Army Special Operations Forces (ARSOF) Enlisted and Officer Commitment and
Volunteer Statement (FY16-FY17)
1. I willfully volunteer and commit for Army Special Operations Forces (ARSOF) training under the
provisions of DA Pam 600-3 (Officers) or Chapter 5, AR 614-200 (Enlisted). I acknowledge that I
have met all criteria listed in DA Pam 600-3 (Officers) or Chapter 5, AR 614-200 (Enlisted), as well
as all applicable MILPER Messages. I commit to properly informing my Chain of Command of my
intent and will inform them of any changes to my Assessment and Selection (A&S) report date. If
not already airborne qualified, I volunteer for airborne training and understand that failure to
successfully complete airborne training will disqualify me from ARSOF training and duty.
2. Upon successful completion of A&S, I request branch transfer from _____________ to an
ARSOF Branch upon graduation from the Regimental SF/CA/PO Officer Qualification Course.

Soldiers on assignment may not be allowed to attend A&S without their branch’s prior approval
(DA Form 4187). Soldiers who willingly commit for ARSOF training prior to receiving assignment
notification will be deferred to allow ARSOF A&S attendance. For A&S graduates, assignment to
the ARSOF Qualification Course will take precedence over any assignment conflict. Stabilization of
current drill sergeants and detailed recruiters will not be broken.
