SUBJECT: Psychological Operations Volunteer Statement (CMF 37 Enlisted)
1. I hereby volunteer for Psychological Operations training. If not already airborne qualified, I volunteer for airborne
training and understand that failure to successfully complete airborne training will disqualify me from PSYOP training
and duty.
2. Upon successful completion of the CMF 37 Qualification Course (CMF 37 QC) and prior to my departure from my
losing command, I agree to reenlist or extend my enlistment to meet the 36 month remaining service obligation; one
year in the training pipeline and two with the operational group.
3. Are you a U.S. Citizen? Yes ____No____ Non US citizens or personnel that hold dual citizenship are ineligible
for a Secret Security clearance and may not attend PSYOP training.
4. Are you currently on assignment or have you received notification of assignment? Yes ____No____

assignment may not be allowed to attend CMF 37 QC without their branch’s prior approval (DA Fm 4187). Soldiers
who volunteer for CMF 37 QC prior to receiving assignment notification will be deferred to allow PSYOPQC
attendance. For CMF 37 QC graduates, assignment to the CMF 37 QC will take precedence over any assignment
conflict. Stabilization of current drill sergeants and detailed recruiters will not be broken.
5. Have you been convicted by a court-martial or have disciplinary action under UCMJ (Article 15) in your official

6. Soldiers that have disciplinary action noted in their official military personnel file may not apply. This provision
can only be waived by the Commanding General, United States Army Special Warfare Center and School on a case
by case basis.

RANK SPC-SSG. PROMOTABLE SSG’S AND NON PROMOTABLE/PROMOTABLE SFC'S
ARE NOT ELIGIBLE; Soldiers cannot have more than 14 years AFS
Enlisted Record Brief (ERB)
Tech Score or 101 to 105 or higher (depending on the test date)
DLAB score 85 or higher. You are required a DLAB even if you have a DLPT rating
(study material can be located at www.deltagear.us/Kit&Tools/Kit/DLAB.htm ) or take
a practice quiz at http://www.navy.com/careers/information-andtechnology/cryptology.html .
DLPT Rating, If Proficient in Any Other Language, with a 1/1 or higher is acceptable.
Airborne Qualified Or Willing To Volunteer For Airborne Training
Pass SERE Physical IAW AR 40-501 IF NOT AIRBORNE QUALIFIED, physical must
also be stamped for Airborne. Submit original DD Form 2808 and DD Form 2807-1
with all lab work to include EKG. Physical must be annotated “qualified for SERE
training” (DD Form 2808, block 74a).
APFT Card With-in 6 Months of Assessment and Selection with height and weight.
Include DA 5500/5501 if you need to be taped.
Interim Secret Clearance. Security Clearance Verification is usually on the ERB, if not, a
copy of your JPAS from your S2 verifying your level of clearance.
Please QC your packet and ensure you have everything prior to submitting. We will not accept
incomplete packets. Packets can be e-mailed to the following:
